氨溴索预防外伤性血气胸剖胸术后并发症的疗效观察

摘　　要：目的:观察盐酸氨溴索注射液预防外伤性血气胸剖胸术后肺部并发症的临床疗效和安全性。方法:43例外伤性血气胸剖胸术后患者随机分为观察组23例和对照组20例。对照组患者术后常规保持呼吸道通畅,给予吸氧、止血、止痛、抗感染、预防应激反应、补液等常规治疗。观察组患者术前给予盐酸氢溴索注射液30 mg加0.9%氯化钠注射液100 ml,ivd qd,术后常规治疗同对照组,予盐酸氨溴索注射液30 mg加0.9%氯化钠注射液100ml,ivd tid。术后疗程均为6 d。监测术后第7天两组氧合指数、血清降钙素原(PCT)、肺部感染及肺不张发生率,观察两组治疗期间药品不良反应发生情况。结果:治疗后,观察组氧合指数、PCT>0.5 ng·ml^(-1)例数等指标均优于对照组,肺部感染及肺不张发生率低于对照组,差异有统计学意义(P<0.05)。两组均未见明显不良反应发生。结论:氨溴索能降低外伤性血气胸剖胸术后患者的肺部并发症。Objective：To observe the thoracotomy clinical efficacy and safety of ambroxol hydrochloride injection on the prevention of traumatic pneumothorax pulmonary complications.Methods：Forty three cases of traumatic hemopneumothorax patients were randomly divided into the observation group with 23 patients and the controlled group with 20 cases. The observation group was given preoperative ambroxol hydrochloride injection 30mg plus 0.9% sodium chloride injection 100ml, ivd, qd, after operation, based on the conventional treatment in the controlled group ambroxol hydrochloride injection 30mg plus 0.9% sodium chboride injection 100ml, ivd tid. The patients in the the controlled group were conventionally treated： keeping the airway open, giving oxygen, hemostatic, analgesic anti infection, prevention of stress response, rehydration and other conventional treatment. The postoperative course was 6d. The oxygenation index, procalcitonin （PCT）, pulmonary infection and incidence rate of pulmonary atelectasis were measured on the 7th day in both groups.Methods：The oxygenation index and PCT （〉0.5 ng·ml-1） of the observation group improved more than those in the controlled group. The incidence rates of pulmonary atelectasi and pulmonary infection in the observation group were lower than those in the controlled group. There was statistically significant difference（P〈0.05）.Conclusion：Ambroxol hydrochloride injection could reduce the thoracotomy pulmonary complications resulting from traumatic pneumothorax.
