摘 要:目的评价三维斑点追踪成像(3Dspeckletrackingimaging,3D-STI)技术检测不同年龄正常成人左心室心肌纵向、圆周向、径向和面积应变特征,并探讨其与左心室射血分数的相关性。方法将98例健康成人按其年龄分为青年组(年龄18~44岁,33例)、中年组(年龄45~64岁,34例)和老年组(年龄≥65岁,31例),使用3D-STl分析软件得到左心室心肌应变值:包括基底段、中间段和心尖段各应变值,各整体应变值以及左心室射血分数。结果3D-STI所测左心室17节段分析示各年龄组纵向、圆周、径向及面积应变在中间段最大,基底段最小(径向应变在心尖段最小);与中年组比较,老年组各整体应变值呈下降趋势(P〈O.05);各年龄组整体应变值与左心室射血分数相关性比较中,面积应变与射血分数相关性最大,r值分别为-0.785,-0.867,-0.840(P〈O.01)。结论正常成人左心室不同节段应变值之间有一定的变化规律;随着年龄的增长,左心室各应变值有降低趋势;面积应变与左心室射血分数相关性最好。Objective To evaluate the ability of 3D speckle tracking imaging (3D-STI) for assessing left ventricu lar(LV) longitudinal strain(LS),circumferential strain(CS),radial strain(RS) and area strain(AS) in normal adults with different a!. To find out the correlation between global strain(GLS, GCS,GRS,GAS) and the left ventricular ejection fraction(LVEF). Methods A total of 98 healthy adults were divided into young people group (18~44 years old,n=33) ,middle-aged group(45~ 64 years old,n= 34) and elderly group(~65 years old, n= 31). All volunteers were examined by 3D-STI,the software was applied to acquire the data including the average strain in apex,mid and basal segment,the global strain and the LVEF. Results Left ventricular wall was divided into 17 segments,the LS, CS,RS and AS were largest at mid segment and smallest at basal segment(the RS was smallest at apex segment) . Compared with the middle-aged group, the elderly group had lower GLS, GCS,GRS and GAS(P〈0.05). The largest of correlation between global strain and LVEF was GAS, r value was -0. 785, -0. 867, -0. 840 respectively(P〈 0.01). Conclusions There is a certain regularity in the strain at different level of left ventricular. With the age grow ing,the strain of the left ventricle decrease. Among the four global strains,GAS has the best correlation with the left ventricular ejection fraction.
