摘 要:目的探讨声触诊弹性图像灰度定量技术在乳腺良恶性病变鉴别诊断中的应用价值。方法选取我院行穿刺活检或手术切除的乳腺病变患者66例,共70个乳腺结节进行了弹性图灰度定量分析,采用ROC曲线分析该方法在鉴别诊断乳腺良恶性病变的敏感性和特异性。结果乳腺良性、恶性病变的弹性图平均灰度值分别为68.72±39.88和25.26±11.35,两者比较差异有统计学意义(P<0.001),ROC曲线显示:当诊断阈值为32.13时,弹性图灰度值鉴别诊断良恶性病变的敏感性和特异性分别为94.1%和85.7%,曲线下面积为0.918±0.038。结论弹性图像的灰度定量方法可有助于乳腺良恶性病变的鉴别诊断。Objective To assess the application of gray-level quantification method of acoustic radiation force impulse (ARFI) in differential diagnosis of breast lesions. Methods Sisty-six patients with breast lesions (70 lesions) underwent the gray-level quantification analysis. All breast lesions were confirmed histologically by means of surgery or biopsy. The sensitivity and specificity of this method were assessed by ROC curve analysis. Results The mean gray-levels of benign and malignant lesions were 68.72±39.88 and 25.26±11.35, respectively, there was significant difference(P〈0.001 ). When the cut-off value was 32.13, this method can predict malignant lesions with a sensitivity of 94.1% and specificity of 85.7%. It gave an area under the curve of 0.918-+0.038. Conclusion Gray-level quantification method of the ARFI imaging is useful in differential diagnosis of breast lesions.
