摘 要:目的探讨罗哌卡因聚乙二醇–聚乳酸(Rop-PEG-PLA)微球对切口痛模型大鼠坐骨神经阻滞的长期作用。方法 80只雄性Wistar大鼠随机分为4组(每组20只),分别于坐骨神经周围间隙给予0.9%NaCl溶液0.2 mL(对照组),PEG-PLA微球10 mg(空白微球组),0.5%罗哌卡因0.2 mL(含罗哌卡因1 mg,罗哌卡因组)或RopPEG-PLA微球10 mg(含罗哌卡因1 mg,罗哌卡因微球组),之后均按照Brennan法制作大鼠切口痛模型。观察术前24 h和术后2 h、4 h、8 h、1 d、2 d、3 d、5 d和7 d时大鼠行为学的变化,免疫组织化学方法检测术后2 h和2 d时脊髓背角c-fos的表达,HPLC法检测大鼠血浆中罗哌卡因的浓度。结果对照组行为学及c-fos表达与空白微球组的差异无统计学意义(P>0.05)。术后2 h时,罗哌卡因组、罗哌卡因微球组行为学阈值与对照组相比均增高(P<0.05),c-fos表达均减少(P<0.05)。罗哌卡因微球组与罗哌卡因组相比,其行为学阈值增高时间(3 d vs2 h)及测得血浆药物时间(8 h vs 4 h)均延长,术后2 d时c-fos表达减少(P<0.05)。结论 Rop-PEG-PLA微球可延长切口痛模型大鼠坐骨神经阻滞镇痛时间达3 d。Objective To investigate the efficiency of ropivacaine PEG-PLA microspheres for sciatic nerve block in rats. Methods Eighty Wistar rats were randomly divided into four groups (n --20, each) and were treated with 0.2 mL of 0.9% saline( control group), 10 mg of PEG-PLA( PEG-PLA group), 0.2 mL of 0.5% ropivacaine( ropiva- caine group) and 10 mg of 10% Rop-PEG-PLA microspheres( Rop-PEG-PLA group), respectively. The incision pain models were produced as described by Brennan. Mechanical thresholds and thermal latencies were measured at 24 h before incision and at 2, 4, 8 h, as well as 1,2, 3, 5 and 7 d after incision. Expression of c-fos protein in the spinal cord was determined by immunohistochemistry at 2 h and 2 d after incision. The ropivacaine concentration in plasma of ropivacaine group and Rop-PEG-PLA group was detected by HPLC analysis. Results Behavioral testing and c-fos expression in PEG-PLA group showed no significant difference compared with those in control group ( P 〉 0.05 ). The thresholds and latencies in ropivacaine group and Rop-PEG-PLA group were significantly higher than those in control group at 2 h after incision( P 〈 0.05 ). Expressions of c-fos protein were suppressed in ropivacaine group and Rop-PEG-PLA group at 2 h after incision, compared with that in control group ( P 〈 0.05 ). The duration of anti-hyperalgesia effect ( 3 d vs 2 h) and ropivacaine concentration in plasma ( 8 h vs 4 h) in Rop-PEG-PLA group were longer than those in ropivacaine group, and c-fos expression in Rop-PEG-PLA group was lower than that in ropivacaine group at 2 d after incision ( P 〈 0.05 ). Conclusion Single administration of ropivacaine PEG-PLA microspheres for sciatic nerve block provides effective analgesia for postoperative pain for 3 d.
