应用超声闪烁伪像预测一水草酸钙结石的初步研究

摘　　要：目的定量评价超声闪烁伪像(TA)对一水草酸钙(COM)结石的预测价值。方法采用相同的体外实验条件,超声扫查25颗COM结石(COM组)与25颗非COM结石(非COM组),分别测量结石TA的宽度(TAW)、高度(TAH)和强度(TAI),分析参数间的相关性并绘制受试者工作特征(ROC)曲线。结果 COM组TAW、TAH及TAI均低于非COM组,差异有统计学意义(P均<0.01);ROC曲线分析显示,以1 661为截断值时,TAI诊断COM结石的曲线下面积为0.80,诊断敏感性84.0%,特异性76.0%,准确率80.0%。结论 TA有助于预测COM结石,为泌尿系结石患者选择合适的治疗方法提供参考,该方法有重要的临床意义和应用前景。Objective To assess the value of twinkling artifact （TA） in quantitative prediction of calcium oxalate monohydrate （COM） stones. Methods Twenty-five COM stones （COM group） and twenty-five stones of other chemical compositions （non-COM group） were included in this study. All urinary stones were studied in vitro with color Doppler ultrasound. The twinkling artifact width （TAW）, height （TAH） and intensity （TAI） were recorded. The re- lationships between the data were analyzed statistically, followed by the illustration of receiver operating characteristic （ROC） curve. Results TAW, TAH and TAI of COM group were obviously smaller than those of non-COM group （ all P 〈0.01 ）. The ROC curve displayed that the cutoff value of TAI at 1 661 could be used to diagnose COM stones. The sensitivity, specificity, accuracy and area under the curve were 84.0%, 76.0%, 80.0% and 0.80, respectively. Conclusion TA may be helpful in the detection of COM stones and instruction of stone treatment.
