摘 要:目的胰岛素样生长因子-1(insulin like growth factor-1,IGF-1)具有类似胰岛素的代谢作用和促有丝分裂作用,在糖尿病微血管并发症的作用备受关注。文中初步探讨2型糖尿病患者视网膜病变与IGF-1水平的相关性。方法检测90例2型糖尿病患者的血IGF-1、空腹血糖(fasting plasma glucose,FPG)、糖化血红蛋白(Hemogiobin A1C,HbA1c)、三酰甘油(triglyceride,TG)、总胆固醇(total cholesterol,TC)、低密度脂蛋白胆固醇(low density lipoprotein-cholesterol,LDL-C)及高密度脂蛋白胆固醇(high density lipoprotein-cholesterol,HDL-C)水平。其中47例合并视网膜病变作为试验组,43例无视网膜病变为对照组。IGF-1检测采用酶联免疫吸附法(ELISA)。结果试验组的IGF-1水平与对照组相比明显增高(4.16±4.07 vs2.79±2.48,P<0.05);试验组HbA1c、FPG、病程分别为(9.35±1.97)%、(8.31±2.87)mmol/L和(12.79±7.90)月,较对照组[(8.21±1.85)%、(7.46±3.05)mmol/L和(8.96±6.59)月]亦明显增高(P<0.05)。Spearman相关性分析显示IGF-1与病程、HbA1c、FPG均呈正相关(P<0.05);多元线性回归分析结果显示,病程、HbA1c是IGF-1的影响因素。结论 2型糖尿病合并视网膜病变患者血清IGF-1水平明显升高,提示IGF-1可能参与了糖尿病视网膜病变的发生发展。Objective Insulin-like growth factor-1 (IGF-1) has the metabolism and mitogenic effects similar to insulin, and correlates with the diabetic microangiopathy. The aim of this study is to investigate the correlation of diabetic retinopathy in type 2 diabetes patients and serum IGF-1.MethodsSerum levels of IGF-1, fasting plasma glucose (FPG), hemogiobin A1C (HbA1c), triglyceride (TG), total cholesterol (TC), low density lipoprotein-cholesterol (LDL-C), and high density lipoprotein-cholesterol(HDL-C) were tested in 90 cases with type 2 diabetes. They were divided into two groups: type 2 diabetes patients with diabetic retinopathy (experimental group) and patients without any vascular complications (control group). IGF-1 is detected by enzymelinked immunosorbent assay (ELISA).ResultsCompared with the control group, the level of IGF-1 in the experimental group was significantly increased (4.16±4.07 vs 2.79±2.48, P〈0.05). The levels of HbA1c ([9.35±1.97]%, FPG (8.31±2.87 mmol/L), and duration (12.79±7.90 month) were also higher in the experimental group than the control group ([8.21±1.85])%, [7.46±3.05]mmol/L) and (8.96±6.59)month (P〈0.05). Spearman regression analysis showed that the IGF-1 was correlated with the FPG and HbA1c (P〈0.05). Multiple linear regression analysis showed that the course of disease and HbA1c were influencing factors of IGF-1. ConclusionThe serum level of IGF-1 in type 2 diabetes patients with retinopathy was increased obviously, suggesting IGF-1 playing an potential important role in the pathogenesy of diabetic retinopathy.
