降调节过程中卵巢黄体囊肿形成的临床观察

摘　　要：目的探讨降调节过程中卵巢黄体囊肿形成的影响因素。方法回顾性分析348例采用标准长方案促排卵的不孕患者在排卵后4～5d、排卵后6-7d、排卵后8～10d开始降调节与卵巢黄体囊肿形成的关系。根据降调节开始时间不同分成三组：A组（排卵后4～5d开始降调节，n=217）、B组（排卵后6-7d开始降调节，n=103）、C组（排卵后8-10d开始降调节，n=28）；根据患者降调节过程中是否出现卵巢黄体囊肿分为两组：N1组（囊肿组，n=99）、N2组（非囊肿组，n=249）。结果C组卵巢黄体囊肿形成率（50．00％）显著高于A组（28．11％）和B组（23．30％）（P〈0．05）；囊肿组患者（N1组）降调节日雌二醇（E2）水平显著高于非囊肿组患者（N2组）（801．5±389．1pmol／L vs．721．6±319．2pmol／L，P〈0．05），而囊肿组患者（N1组）降调节日孕酮（P）水平显著低于非囊肿组患者（48．2±20．3nmol／L vs．53．6±21．6nmol／L，P〈0．05）。结论排卵后6～7d开始降调节卵巢囊肿形成率较低，囊肿形成率受降调节日E2、P水平影响。Objective： To explore the factors of influencing ovarian cyst formation. Methods： The data of 348 IVF cycles with different initiating time of pituitary down-regulation,i, e. 4- 5 days,6-7 days or 8-10 days after ovulation were evaluated retrospectively. The patients were divided into 3 groups according to initiating time of pituitary down-regulation. Among them, 217 patients in group A, 103 patients in group B and 28 patients in group C started pituitary down-regulation at 4-5 days,6-7days,8- 10 days after the ovulation respectively. Then all the patients were divided into 2 groups according to whether ovarian cyst formed, with 99 patients with ovarian cyst in group N1, and 249 patients without ovarian cyst in group N2. Results： The cyst formation rate in group C（50.00%）was significantly higher than that in group A （28. 11%）and group B（23.30%）（P〈0. 05）. The estradiol levels in N1 group were significantly higher than those in N2 group[（801.5±389.1）pmol/L vs. （721.6-5319.2）pmol/L,P〈0. 05]. The progesterone levels in group N1 were significantly lower than those in group N2[（48.2±20.3）nmol/L vs. （53.6±21.6） nmol/L,P〈0.05]. Conclusions： The cyst formation rate is influenced by estradiol and progesterone levels during the pituitary down-regulation,which is lower when pituitary down-regulation is initiated around 6-7 days after the ovulation.
