甲磺酸α-二氢麦角隐亭及甲磺酸溴隐亭治疗高催乳素血症的疗效和副作用的比较

摘　　要：目的探讨甲磺酸溴隐亭及甲磺酸α-二氢麦角隐亭治疗高催乳素血症的疗效及副反应。了解甲磺酸α-二氢麦角隐亭的临床应用价值。方法募集2010年3月至2012年6月在广西钦州市第二人民医院妇产科门诊就诊的高催乳素血症患者96例,随机分为两组:观察组(A组,n=48)服用甲磺酸α-二氢麦角隐亭;对照组(B组,n=48)服用甲磺酸溴隐亭。分别于服用药物后的30、60、90、120、150、180d测定血清催乳素(PRL)下降的情况,并观察月经恢复情况、溢乳控制情况及药物副反应。结果 (1)两组患者使用药物后血清PRL水平均下降,但组间无统计学差异(P>0.05)。(2)观察组头晕、恶心和体位性低血压等副反应发生率显著低于对照组(P<0.05)。对照组有5例因严重副反应停药,改用甲磺酸α-二氢麦角隐亭治疗。观察组中无因为药物副作用而终止治疗者。结论服用甲磺酸α-二氢麦角隐亭治疗高催乳素血症的临床疗效与甲磺酸溴隐亭相似,但其药物副作用的发生率及严重程度明显低于使用甲磺酸溴隐亭。甲磺酸α-二氢麦角隐亭治疗高催乳素血症是临床用药的一种新选择。Objective： To compare the efficacy and side effect between dihydro-α-ergocryptine mesylate and bromocriptine methanesulfonate in treatment of hyperprolactinemia. Methods： A total of 96 patients with hyperprolactinemia were recruited in our clinic from March 2010 to June 2012. The patients were randomly allocated into two groups：dihydro-α-ergocryptine mesylate group （group A）and bromocriptine methanesulfonate group（group B）with 48 patients in each group. The decline of serum prolactin（PRL）levels, recovery of menstruation,galactorrhea control, as well as side effect were observed at 30,60,90,120,150,180 days after taking medicine. Results. The PRL levels were decreased in both groups in the same pattern after taking the medicines,without statistically difference（P〉0.05）. The side effects in dihydro-α-ergocryptine mesylate group were significantly lower than those in the bromocriptine group（P〈0.05）. Five patients in control group shifted to take dihydro-α-ergocryptine mesylate due to the intolerable side effects. Conclusions： There is no significant difference in the efficacy of treatment of hyperprolactinemia between dihydro-α-ergocryptine mesylate and bromocriptine. However, the frequency and severity of side effects of dihydro-α-ergocryptine mesylate are significantly lower than bromocriptine. Dihydro-α- ergocryptine mesylate is a new option for physicians for treating hyperprolactinemia.
