不同时间预注小剂量顺式阿曲库铵对琥珀酰胆碱引起的肌颤搐的抑制效果

摘　　要：目的观察不同小剂量的顺式阿曲库铵在不同时间预注对琥珀胆碱肌引起的肌颤搐的抑制效果。方法60例择期全麻女性患者根据不同预处理药分成2大组,即3分钟预注顺式阿曲库铵组(A组:0.005mg/kg,0.01mg/kg,对照组生理盐水);4分钟预注顺式阿曲库铵组(B组:0.005mg/kg,0.01mg/kg,对照组生理盐水)。预处理后静脉注射琥珀胆碱1.5mg/kg。监测肌松情况TOF T1肌颤搐抑制95%时行快诱导气管插管。观察琥珀酰胆碱引起的肌肉震颤、肌松程度、起效时间。结果对1.5mg/kg的琥珀酰胆碱引起的肌颤搐0.005mg/kg 3分钟组无抑制作用(P>0.05),0.005mg/kg 4分钟组、0.01mg/kg 3分钟,0.01mg/kg 4分钟组有抑制作用(P<0.01)。结论顺式阿曲库铵小剂量0.005mg/kg 4分钟,0.01mg/kg 3分钟、4分钟预处理能有效消除琥珀胆碱引起的肌震颤。Objective This survey was made to observe the inhibition effects of succinylcholine fasciculation following pretreatment with low dose cisatracurium on the different time. Methods A total of 60 female patients were divided into 6 groups ： 0.9 M NaCl （ AC）, 0. 005 mg/kg atracurium （ Group A1 ）, 0.01 mg/kg atracurium （Group A2 ） pretreated 3 minutes before 1.5mg/Kg succinylcholine intravenous injection, 0.9% NaCl （Group BC）, 0. 005 mg/kg atracurium（Group B1 ）, 0.01 mg/kg atracurium（Group B2） pretreated 4 minutes before 1.5mg/Kg succinylcholine intravenous injection. Neuromuscular monitor was used to evaluate the fasciculations,muscular blocking effects and onsettime were observed. Results Muscle fasciculations were inhibitied among Group 0. 005mg/Kg 4 minute, 0.01mg/Kg 3 minute, 0.01mg/Kg 4 minute（P〈0.01）. Conclusion Pretreatment with cisatracurium 0. 005mg/Kg 4 minutes,0.01mg/Kg 3 minutes and 4 minutes were effective to prevent muscular fasciculation following succinyl-choline injection.
