原发性高血压患者血清胱抑素C与心脏重构的关系

摘　　要：目的研究原发性高血压(EH)患者血清胱抑素C(Cys C)与心脏重构的相关性。方法纳入收缩功能正常(LVEF>50%)的EH患者200例为试验组,纳入正常体检者50例为对照组,对上述对象进行血清CysC浓度检测并行超声心动图检查,测量左心房内径(LAD)、左心室舒张末期内径(LVDD)、室间隔厚度(IVS)、左心室后壁厚度(LVPW),并计算左心室重量指数(LVMI);对照组上述指标正常,试验组根据血清CysC中位数浓度(1.05 mg/L)分为CysC升高组(98例)及CysC正常组(102例)。结果试验组较对照组血清CysC浓度明显升高(P<0.05)。CysC升高组的LAD、LVDD、IVS、LVPW、LVMI较CysC正常组显著增高(P<0.05)。CysC含量与LAD、LVDD、IVS、LVPW、LVMI呈正相关关系(相关系数分别为0.422、0.381、0.367、0.256、0.350,P<0.05),血清CysC浓度是心脏重构的显著影响因素。结论原发性高血压患者血清CysC浓度与心脏重构相关,对高血压心脏重构的程度及预后判断有一定预测价值。Objective To study the correlation between the level of serum Cystatin C and cardiac remodeling in patients with essential hypertension（EH）. Methods Two hundred patients diagnosed as EH with normal heart function and 50 normal controls who had measurement of CysC were enrolled in this study.Then the group of patients with essential hypertension, whose LAD, LVDD, IVS, LVPW were measured using echocardiography and LVMI was evaluated, were divided into two groups based on the serum Cystatin C level, Results The serum CysC level in patients with hypertension was higher than that in control group（P〈0.05）. With the increase of serum CysC levels, the mesurement of LAD, LVDD, IVS, LVPW increased.Multivariable stepwise regression analysis showed that CysC had significant correlation with LAD, LVDD, IVS, LVPW. With the increase of serum levels of CysC, the cardiac remodeling was more obvious. Conclusion The serum level of CysC was positively related with cadiac remodeling, the concentration of serum CysC may be a promising predictor of cardiac remodeling and prognosis in EH.
