小剂量硬膜外麻醉在老年腹膜炎手术中的应用(附24例分析)

摘　　要：老年弥漫性腹膜炎就诊时病情多较危重,常有不同程度的毒血症表现,并有水电解质紊乱和酸碱平衡失调,而且需要紧急手术。此类病人的麻醉一般认为插管全麻为妥。我院1988年1～8月共24例老年弥漫性腹膜炎剖腹手术全施行小剂量持续硬膜外麻醉,现总结如下:From January to August 1988, 24 cases of aged over 60 years patients with diffuse peritonitis underywent an exploratory laparotomy under small dosage of lidocaine persistently epidural anesthesia in our hospital. 2% lidocaine was chosen as the anesthetic agent,About 7 ml was the initial dosage, the range of the dose was 2 ml to 10 ml. Dolantin was given 30 mg to 50 mg as an adjunant routinely. For the shock patient operation was done under local anesthesia initially, persistently epidural anesthesia was instituted only after the blood pressure and respiration rate return to normal. In this series, their blood pressure and respiraion rate were descended in different degree. during the process. Pulse was not changed significantly. No severe complications and anesthetic accidence occurred during the operation. Anesthetic effect was satisfactory. The results of this series show that the epidural anesthesia is an useful anesthetic procedure for the opertion of the aged patients with diffuse peritomitis. Blood pressure was monitored throughout the course of anesthesia.
