股前外侧穿支皮瓣游离移植修复四肢大面积皮肤缺损的疗效分析

摘　　要：目的探讨股前外侧穿支皮瓣游离移植修复四肢大面积皮肤缺损的疗效。方法选择本院2010年3月－2012年6月收治的四肢皮肤软组织缺损患者42例，均进行股前外侧穿支皮瓣游离移植修复，选择同期四肢皮肤软组织缺损患者42例为对照组，进行传统择期腹部或小腿后部植皮治疗，术后随访观察皮瓣存活情况、血运及感觉情况。结果研究组手术时间长于对照组，但是住院时间短于对照组，治疗费用少于对照组，组间比较，差异有统计学意义；研究组皮瓣存活率、保护性感觉恢复率优于对照组，组间差异有统计学意义。研究组患者皮瓣血运及外形良好，未出现皮瓣坏死及感染情况。结论股前外侧穿支皮瓣游离移植修复四肢大面积皮肤缺损，皮瓣血运良好，存活率高，修复外形好，是治疗四肢大面积皮肤缺损的理想方法。Objective To investigate the clinical application of anterolateral thigh flap transplantation in patients with large skin defect. Methods 42 patients with skin and soft tissue defects of extremities admitted tour hospital from March 2010 to June 2012 were selected, and all were given anterolateral thigh flap transplantation treatment. And 42 patients in the same period were selected as control group, they were given traditional elective abdominal or calf skin graft treatment, and postoperative flap survival , blood circulation and feeling situation were observed. Results The operative time of study group is longer than the control group , but hospital stay was shorter than control group, the cost of treatment is less than control group, the difference between two groups was statistically significant ; Flap survival rate, the protective sensation recovery rate of the study group were better than the control group, difference between the two groups was significant. The patients of study group were all with good flap blood supply and good shape, and flap necrosis and infection were found. Conclusion Anterolateral thigh flap transplantation in treatment of patients with large skin defect, blood supply of the flap is good, the survival rate is high, repair shape is good and it is the ideal method for the treatment of large skin defect
