摘 要:目的:比较两种不同的匀浆膳在重症患者肠内营养中的应用效果。方法选取86例重症患者按住院号单双号分为观察组和对照组各43例,观察组给予鼻饲立适康系列匀浆膳,对照组按照传统的方法给予鼻饲米糊、瘦肉浆、青菜浆和牛奶等做成的匀浆膳,比较两组重症患者的营养状况。结果两组患者在肠内营养支持10 d后Hb、白蛋白( ALB)和空腹血糖( FBS)比较,差异无统计学意义(t值分别为0.34,0.58,0.41;P>0.05),淋巴细胞总数(TLC)比较差异有统计学意义(t =2.29,P<0.05),总蛋白(TP)比较差异有统计学意义(t=3.26,P<0.01);治疗20 d后观察组Hb、TLC、TP、ALB分别为(13.55±3.31)g/L,(17.28±2.67)×108/L,(63.12±4.08)g/L,(37.46±6.12)g/L,高于对照组的(11.29±3.35)g/L,(10.46±5.49)×108/L,(57.38±4.84)g/L,(30.15±4.22)g/L,两组比较差异有统计学意义(t值分别为3.15,7.39,5.95,4.36;P<0.01),FBS两组比较差异无统计学意义(t=0.53,P>0.05)。结论鼻饲立适康系列匀浆膳比鼻饲传统制作的匀浆膳能够更好地改善重症患者的营养状况。Objective To compare the clinical effect of two kinds of homogenate catering on enteral nutrition ( EN ) in critically ill patient .Methods Eighty-six critically ill patients were divided into the observation group and the control group according to the odd and even number of hospitalized .The observation group received the nasogastric series of homogenate catering from Lishikang , and the control group received the homogenate catering including rice cereal , lean meat slurry, vegetables pulp, milk, etc.The nutritional status was compared between the two groups .Results After ten days of EN , no differences were found in the levels of Hb, ALB and FBS between the two groups (t=0.34, 0.58, 0.41;P>0.05); the differences were found in the levels of TLC and TP between the two groups (t=2.29, 3.26; P<0.05).After twenty days of EN, the levels of Hb, TLC, TP and ALB were respectively (13.55 ±3.31)g/L, (17.28 ±2.67) ×108/L, (63.12 ± 4.08)g/L, (37.46 ±6.12) g/L in the observation group, which were higher than (11.29 ±3.35) g/L, (10.46 ±5.49) ×108/L, (57.38 ±4.84)g/L, (30.15 ±4.22)g/L in the control group, and the differences were statistically significant (t=3.15, 7.39, 5.95, 4.36, respectively;P<0.01);no difference was found in the level of FBS between the two groups ( t =0.53, P >0.05 ).Conclusions The nasogastric series of homogenate catering from Lishikang is better than the traditional homogenate catering in improving the nutritional status of critically ill patients .
