5-氨基酮戊酸-光动力联合匹多莫德治疗尖锐湿疣临床疗效及对细胞免疫功能的影响

摘　　要：目的观察5-氨基酮戊酸-光动力(5-aminolevulinicacid photodynamic therapy,ALA-PDT)联合匹多莫德治疗尖锐湿疣(condylomata acuminata,CA)患者T淋巴细胞亚群CD3^+、CD4^+、CD8^+、CD4^+/CD8^+的变化及复发率。方法采用流式细胞术检测CA患者198例治疗前后及对照组90例CD3^+、CD4^+、CD8^+、CD4^+/CD8^+的变化,同时观察治疗后的复发率。结果治疗A组100例,痊愈92例,复发8例(8.00%),治疗B组98例,痊愈59例,复发39例(39.80%),2组复发率比较差异有统计学意义(P<0.05);CA患者与对照组比较,CD3^+、CD4^+、CD4^+/CD8^+比值下降,CD8^+升高(P<0.05)。治疗结束后,2组CD3+、CD4^+、CD8^+、CD4^+/CD8^+均较治疗前明显改善(P<0.05),但组间各项指标差异均无统计学意义(P>0.05)。结论细胞免疫功能紊乱及亚临床感染与CA的复发密切相关,ALA-PDT联合匹多莫德治疗CA治愈率高、复发率低。Objective To investigate the effects of 5-aminolevulinic acid photodynamic therapy（ALA-PDT） combined with pidotimod in treatment of condylomata acuminata （CA） changes and recurrence of T cell subsets in patients with CD3＋,CD4＋,CD8＋,CD4＋/CD8＋.Methods The changes of CD3＋,CD4＋,CD8＋,CD4＋/CD8＋ in 198 patients before and after treatment with CA and 90 cases of the control group were detected by flow cytometry,and the relapse rate after treatment was observed.Also,healthy subjects from the body check at the same period in the hospital formed normal control group （n=90） for intergroup comparison.Results In 100 cases from the treatment group,92 cases were cured,8 cases of recurrence（8.00%）,while in 98 cases from the control group,59 cases were cured,39 cases of recurrence （39.80%）,the recurrence rate showed significant difference between two groups （P〈0.05）;compared with normal,CD3＋,CD4＋,CD4＋/CD8＋ in CA patients before treatment decreased,but CD8＋ elevated （P〈0.05）.After treatment,CD3＋,CD4＋,CD8＋,CD4＋/CD8＋ significantly improved than before treatment in both groups （P〈0.05）,but they were no significant changes between two groups （P〉0.05）.Conclusion Cellular immune function disorder and subclinical infection are closely related to CA recurrence,ALA-PDT combined with pidotimod proves high cure rate,low recurrence rate in treatment of CA.
