摘　　要：目的：探讨不含指神经的逆行指动脉岛状皮瓣在修复手指末端皮肤及软组织缺损的临床疗效。方法采用逆行指动脉岛状皮瓣修复手指末端皮肤及软组织缺损48例54指，其中不含指神经28指，带指神经26指，观察修复效果。结果本组54指皮瓣全部成活，无静脉回流障碍，供区创面一期愈合。术后随访6～18个月，皮瓣外形满意、质地柔软、弹性好，色泽与正常接近，皮瓣不臃肿能耐寒，伤指无疼痛，两点辨别觉达6～10 mm。两种术式供皮区外观、弹性、质地与正常指皮肤相似，但26指不含神经的逆行指动脉皮瓣供区两点辨别觉可达4～8 mm，平均5 mm，持物稳定，根据手功能评定试用标准综合评定，优18指，良9指，可1指，优良率达96%。结论不含指神经的逆行指动脉岛状皮瓣是修复手指末端皮肤及软组织缺损一种很好的手术方法，值得推广应用。Objective To discuss the effect of without digital nerve retrograde digital artery island flap in repairing skin and soft tissue of finger end. Methods A total of 54 cases were treated with repairment by reversed digital artery island flap, including 19 with index finger defect, 25 with middle finger defect, and 10 with ring finger defect, which does not contain nerves in 28 cases, with nerves in 26 cases to observe the repair effect. Results All the 54 transferred flaps survived completely. Postopertive follow-up ranged from 6 to 18 months. The donor site without nerve had the appearance of elasticity, texture of the skin was similar to normal, two-point discrimination was average 5mm, hold things steady. According to the TAM standard, the results were excellent in 18 cases, good in 9, fair in 1, excellent and good rate was 96%. Conclusion The repairment of skin and tissue defects in finger end with without digital nerve retrograde digital artery island flap of proper palmar digital nerves is effective, and is worthy widely application.
