应用TwinFix锚钉胫后肌腱重建治疗青年士兵足副舟骨疼痛

摘　　要：目的观察及评价应用TwinFix锚钉胫后肌腱重建治疗足副舟骨疼痛的临床疗效。方法对2009年6月-2012年6月我院收治的足副舟骨疼痛士兵22例25足，全部采用足副舟骨切除、TwinFix锚钉重建胫后肌腱止点手术治疗，术后随访评估患者症状缓解及足部功能恢复情况。结果所有病例术后随访12～24（18．6±3．7）个月。根据Maryland足部评分标准，优20足，良3足，可2足，优良率92％。平均住院（10．1±2．0）d，无切口感染，均恢复日常生活、工作及训练。结论应用TwinFix锚钉胫后肌腱重建治疗足副舟骨疼痛，可明显缓解疼痛症状，相对于传统Kindner手术、单纯副舟骨切除术，应用TwinFix锚钉胫后肌腱重建既可使胫后肌腱止点固定牢靠，抗拉强度高，也可保护足内侧纵弓稳定性，恢复足正常力线，有效防止继发性扁平足，使患者较早恢复足部功能，远期疗效满意。Objective To observe and evaluate the clinical efficacy of posterior tibial tendon reconstruction using TwinFix anchor for painful accessory navicular bone. Methods From June 2009 to June 2012, 25 feet in 22 soldiers with painful accessory navieular bones underwent surgical treatment including accessory navicular bone resection and posterior tibial tendon reconstruction with Twin- Fix anchors. Results All the patients were followed up for 12 to 24 （ 18.6± 3.7 ） months. According to Marcland foot evaluation criteria, efficacy was excellent in 20 feet and good in 3 feet （the other 2 feet were rated as so-so） , with a rate of 92%. The av- erage hospital stay was （ 10.1 ±2. 0） d and no wound infection occurred. All the patients resumed the normal training after. Conclusion Posterior tibial tendon reconstruction with TwinFix anchors can significantly relieve the pain among the young soldiers with painful accessory navicular bones. Opposed to traditional Kindner surgery and simple excision, the application of TwinFix an- chors to posterior tibial tendon reconstruction not only enables the tendon firmly fixed with high tensile strength, but also protects the stability of the medial longitudinal arch and restores the normal line of force sufficient to preventing from secondary flat feet. It helps the patients to restore foot function earlier, and the long-term outcome is satisfactory.
