老年期脊柱结核的影像学特点分析

摘　　要：目的探讨老年患者脊柱结核的影像学特点和形成原因。方法回顾性分析55例经病理证实[手术(25例)、穿刺活检(30例)]的老年期脊柱结核患者影像资料,归纳影像学特点。其中X线检查55例,49例患者行CT扫描,32例行MR扫描。结果老年期脊柱结核影像学上表现为:(1)椎体弥漫性骨质破坏,死骨形成及骨质增生不明显;(2)脊柱结核的病理性骨折影像学上表现为椎体水平压缩或呈"鱼口状";(3)脊柱结核所致软组织肿块小,且少有钙化;(4)老年期脊柱结核椎间盘病变轻。结论掌握老年患者脊柱结核影像学特点,有助于提高正确诊断率。Objective To explore the radiological image features of spinal tuberculosis in senile patients. Methods Imaging findings in 55 senile patients with spinal tuberculosis confirmed by pathological examination （25 patients underwent surgical operation and the other 30 ones underwent biopsy） were analyzed retrospectively. All the 55 patients underwent X-ray exami- nation. Moreover,49 patients underwent plain CT scanning,and 32 ones MRI. Results The radiological image features of spinal tuberculosis in senile patients consisted of general destruction of vertebral body, fracture of spinal bones showing level oppression, small soft tissue mass with little calcification, and gentle pathological destruction in discs. Conclusion Senile spinal tuberculosis shows distinctive imaging features, which are helpful for doctors to make a correct diagnosis.
