摘　　要：目的:规范化运作健康管理家园平台,探讨其对高血压患者血压及血脂控制的影响。方法 :全科医生进行全方位健康管理,半年后比较血压及血脂(TC、TG、LDL-C、HDL-C)控制情况。结果 :管理后,血压及血脂明显降低(P<0.05),血压达标者血脂较血压未达标者明显降低(P<0.05)。结论:健康家园一体化管理平台可细化社区病人管理,效果明显。Objective： This study explored the effect of operation of health management on blood pressure and scrum lipid control of hypertensive patients. Methods： Three hundred patients were enrolled in the health management.The blood pressure and serum levels of triglyceride（TG）, total cholesterol（TC）, lipoprotein cholesterol （LDL-C）, high density lipoprotein cholesterol （HDL-C） were measured and compared before and after standardized operation. Results： After operation of health management the blood pressure and serum lipids descended obviously （P〈0.05） and the decrease of serum lipids was more obvious in the patients with standard blood pressure than with uncontrolled blood pressure （P〈0.05）. Conclusion： Community health management is of benefit to the control of blood pressure and lipids in the patients with hypertension.
