盐酸吗啡控释片直肠给药控制癌痛临床观察

摘　　要：目的：观察盐酸吗啡控释片（美菲康）直肠给药治疗中重度癌痛的临床疗效。方法将114例中重度疼痛癌症患者随机分为直肠给药和口服给药2组，各57例，分别采用盐酸吗啡控释片直肠给药和口服给药，美菲康10 mg为开始剂量，每12 h给药1次，之后根据镇痛效果调整剂量，最小日剂量20 mg，最大日剂量240 mg。结果直肠给药组有效率为94．7％，口服给药组有效率为96．5％，2组相比差异无统计学意义（P＞0．05）。2组不良反应相近。结论盐酸吗啡控释片直肠给药是一种安全、有效的镇痛途径，尤其适用于因各种原因导致的不能口服给药的癌痛患者。Objective To explore the clinical effect of morphine hydrochloride sustained -release tablets adminis-tered rectally for moderate to severe cancer pain .Methods 114 cases of moderate to severe pain in cancer patients were randomly divided into two groups of rectal （ rectal group ） and oral （ oral group） administration of of morphine hydrochlo-ride sustained-release tablets （ n=57 each） .Sustained-release tablet of 10 mg was administered as starting dose , 1 dose every 12 h.The dose was adjusted according to the analgesic effect with the minimum daily dose of 20 mg and the maximum daily dose of 240 mg.Results The total effective rate was 96.5%in oral group and 94.7%in rectal group, with no statistically significant difference （P＆gt;0.05）.The side effects were similar between the 2 groups.Conclusion Rectal administration of morphine hydrochloride sustained -release tablets is safe and effective for analgesia .It is espe-cially suitable for patients with cancer pain who can not receive oral administration of morphine hydrochloride sustained -release tablets due to various reasons .
