血液透析滤过对维持性血透患者微炎症与营养状况影响的观察

摘　　要：目的探讨血液透析滤过对维持性血液透析患者微炎症和营养状况的影响。方法选择维持性血液透析的终末期肾病患者160例,随机分为2组:HOF组和HD组,每组各80例,分别于治疗前和治疗3个月后采血检测血清白蛋白(albumin,Alb)、C反应蛋白、白介素6水平。结果 HD组与HOF组患者治疗前血CRP、IL-6和Alb差异均无统计学意义(P>0.05)。治疗后,HD组患者CRP和IL-6水平较治疗前无明显改变(P>0.05);HOF组治疗后血CRP和IL-6水平较治疗前均明显下降(P<0.01)。HD组和HOF组治疗后较治疗前Alb均升高(P<0.05)。HOF组治疗后Alb升高幅度较HD组明显(P<0.05),CRP和IL-6水平下降幅度较HD组大(P<0.05)。结论 HOF治疗维持性血液透析患者可有效清除中大分子炎症因子,减轻微炎症状态,改善MHD患者的营养状况。Objective To study the effect of online hemodiafihration on micro - inflammation and nutritional status in maintenance hemodialysis patients. Methods We collected 160 end - stage renal disease （ESRD） patients on hemodial- ysis,and randomly divided them into two groups ： HOF group （ n = 80, HOF group） and HD group （ n = 80, HD group） . Before and after the treatment for 3 months, the blood of serum albumin （Alb） , C reactive protein （CRP） and interleukin - 6 （ IL - 6 ） level were measured on a biochemistry analyzer. Results No significant differences in CRP, IL - 6 and Alb were found between the two groups before treatment（ P 〉 0. 05 ）. However, CRP and IL -6 significantly decreased after the treatment in HOF group （P 〈 0. 01 ） but not in HD group. Alb significantly increased after the treatment in the 2 groups （P 〈 0.05 ）. After the treatment, Alb was higher in HOF group than in HD group（ P 〈 0. 05 ） ,and CRP and IL - 6 were lower in HOF group than in HD group（ P 〈0. 05 ）. Conclusion HOF can effectively clear the macromolecules inflammatory eytokines, mitigate the micro - inflammatory state, and improve the nutritional status of MHD patients.
