摘 要:目的通过比较妊娠早中期胎儿心脏血流动力学变化,以了解胎儿心脏功能状况。方法采用二维超声+多普勒技术对46例胎龄为10—12周和44例胎龄为16~24周的胎儿心脏进行血流动力学研究。结果妊娠早中期胎心血流动力学存在明显变化:①妊娠中期二尖瓣及三尖瓣口‘e’峰较妊娠早期显著增加,e/a值因而增加;②妊娠中期等容舒张期(IRT)占心动周期的比例较妊娠早期明显减小;③等容收缩期(ICT)在妊娠早期明显可测,至妊娠中期则不能测及;④妊娠中期流出道血流峰速明显增快;⑤妊娠中期胎心率较妊娠早期明显减慢。结论妊娠早中期胎儿心脏功能可用多普勒超声进行无创监测,胎儿心脏收缩及舒张功能随胎龄增加呈改善趋势。Objective To assess fetal cardiac function in the first and second trimester of pregnancy by doppler uhra- sound. Methods Forty - six pregnancies of 10 - 12 weeks gestation and forty - four of 16 - 24 weeks were studied by fetal echocardiography examination. Results Fetal cardiac hemodynamic changed remarkblely from first trimester to sec- ond trimester: (1)'e'wave of inflow- velocity increased sharply in the second trimester, the same as e/a ratio. (2)The cardiac isovolumetric relaxation time(IRT) of the second trimester was shorter than that of the first trimester. (3)The car- diac isovolumetric constraction time(ICT) was measurable in fetus of the first trimester while unmeasurable in the second trimester. (4)the peak out flow velocities increased in the second trimester. (5)The fetal heart rate was slower in the second trimester. Conclusion The fetal cardiac function can be assessed noninvasively by doppler ultrasound. The systolice and diastolic function of fetal heart improved.
