摘　　要：目的评估男性性功能症状评分联合血清睾酮水平检测预测上海市社区中老年男性迟发性性腺功能减退症（late onset of hypogonadism，LOH）的有效性和可行性。方法对2009年11月至2010年6月入组的上海市浦东新区潍坊社区977例40～70岁的中老年男性进行中老年男子雄激素缺乏问卷（ADAM）、老年男子症状问卷（AMS）、欧洲老年男性研究-性功能问卷（EMAS—SFQ）、马萨诸塞州老年男性调查问卷（MMAS）、贝克抑郁量表、SF-36健康调查量表和国际勃起功能问卷调查，同时测定血清中总睾酮（TT）、性激素结合球蛋白（SHBG）水平及游离睾酮（fT）水平。结果研究对象平均年龄为（58．21±7．41）岁，ADAM和AMS筛查LOH的阳性率分别为84．65％和58．65％，男性性功能症状评分筛查LOH阳性率为17．09％；ED阳性率为79．12％，fT水平在LOH患者及正常人中存在统计学差异（P〈0.05）。结论男性性功能症状评分结合测定fr水平能更好地预测中老年男性LOH。Objective To evaluate male sexual symptoms and the level of serum testosterone in predicting the inci- dence of late onset of hypogonadism （LOH） in old and middle-aged males in Shanghai community. Methods 977 cases of old and middle-aged males （40-70 years old） in Weifang community of Pudong area were interviewed at random by trained health workers under a structured questionnaire including ADAM, AMS, EMAS-SFQ, MMAS, SF-36, Beck Depression Inventory and IIEF-5 from Nov. 2009 to June 2010, and the concentration of serum total testosterone, sex hor- mone binding globulin and free testosterone was tested. Results The mean age of the males was （58.21±7.41） years old. The positive rates of LOH screening by ADAM and AMS were 84. 65% and 58.65% respectively, and the positive rate of LOH screening by male sexual symptoms was 17.09%, and the positive rates of erectile dysfunction （ED） was 79. 12%. The level of serum free testosterone in different groups had significant difference. Conclusions The male sexual symptoms combined with the level of serum free testosterone may better predict the incidence of late onset of hypogonadism in old and middle-aged males in Shanghai community.
