老年代谢综合征患者血清25-羟维生素D_3水平变化及临床意义研究

摘　　要：目的探讨老年代谢综合征(MS)患者血清25-羟维生素D3〔25(OH)D3〕水平变化及临床意义。方法入选2010年9月—2012年10月在北京大学人民医院老年医学科住院的年龄≥60岁的MS患者167例(MS组)及同时期非MS对照者187例(对照组)。测量两组患者的血压、身高、体质量,计算体质指数(BMI),并检测空腹血糖(FBG)、血脂、糖化血红蛋白(HbA1c)等指标,采用酶联免疫分析法检测血清25(OH)D3水平。对比分析两组患者上述指标间的差异及血清25(OH)D3与各指标间的相关性。结果 MS组患者血清25(OH)D3水平较对照组降低〔(29.9±6.6)nmol/L与(32.7±7.1)nmol/L,P<0.05〕,两组血清25(OH)D3水平分级(充足、不足与缺乏)间差异有统计学意义(P<0.05)。Pearson相关性分析显示血清25(OH)D3与MS患者的BMI、三酰甘油、FBG及HbA1c呈负相关(r=-0.116、-0.110、-0.118、-0.143,P<0.05)。结论老年MS患者血清25(OH)D3水平较低,且与BMI、血脂、血糖等指标相关。Objective To explore the relationship between serum 25 （OH） D3 level and metabolic syndrome （MS） in elderly. Methods Blood pressure （BP）, height, weight were measured, body mass index （BMI） calculated, fasting blood glucose （FBG）, lipids, glycated hemoglobin （HbAlo） etc. determined in 167 MS patients and 187 controls aged over 60 years, from September 2010 to October 2012 in People＇s Hospital of Peking University. Enzyme- linked immunosorbent assay was used to detect 25 （OH） D level. The difference in the above indicators between 2 groups and the correlation of 25 （OH） D3 with these indicators were analyzed comparatively. Results Serum 25 （OH） D3 level was lower in MS group than in control group （29. 9 6. 6） nmol/L, （32. 7 7.1 ） nmol/L, P 〈 O. 05 , there was significant difference in grading of 25 （OH） D3 level （P 〈0.05）. Pearson correlation analysis showed that serum 25 （OH） D3 was negatively correlated with BMI, triglycerides, FBGand HbA （r = -0.116, -0.110, -0.118, -0.143, P 〈0.05）. Conclusion Serum 25 （OH） D3 level, low in elderly MS patients, is correlated with BMI, lipids, blood glucose and other indicators.
