昆山地区社区卫生服务中心与乡镇医院检验设备的配置现状调查

摘　　要：目的调查江苏省昆山地区社区卫生服务中心与乡镇医院检验设备的配置现状,为进一步探讨小康型乡镇医院检验设备配置标准提供基线数据。方法根据国家关于乡镇卫生院医疗设备配置指导意见设计调查问卷,于2011年8月—2012年2月调查了昆山地区5家市区内社区卫生服务中心,6家中心型乡镇医院和3家一般型乡镇卫生院(乡镇医院)检验设备的配置、运行及维护情况。结果各社区卫生服中心与乡镇医院均配备了全自动生化分析仪、全自动血细胞分析仪、电解质分析仪和尿液分析仪。社区卫生服务中心未配备血气分析仪、血流变仪、酶标仪、洗板机和尿沉渣分析仪;乡镇医院仅未配备血流变仪。社区卫生服务中心配备的30台检验设备中运行良好23台、有缺陷但尚可运行5台、报废2台,乡镇医院90台检验设备中运行良好74台、有缺陷但尚可运行15台、报废1台,差异无统计学意义(χ2=2.849,P>0.05)。社区卫生服务中心的检验设备27台由供应商维护,支付维修费;1台由供应商维护,不支付维修费;乡镇医院分别为87台和2台,差异无统计学意义(χ2=0.149,P>0.05)。社区卫生服务中心28台检验设备中7台进行预防性维护,预防性维护率为25%;乡镇医院89台检验设备全部进行预防性维护,预防性维护率为100%,差异有统计学意义(χ2=80.278,P<0.05)。结论昆山地区乡镇医院检验设备的配置和预防性维护率较社区卫生服务中心好,社区卫生中心在此方面需进一步规范和改进。建议基层医疗机构考虑小型化、便携化、自动化、智能化、标准化的简单微创的用于即时检验的检测设备,配置时应兼顾市场规律和实际需要。Objective To survey the present situation of the allocation of laboratory equipments in community health centers and township hospitals in Kunshan city, to provide baseline data for the further discussion of the well - off township hospi- tal laboratory equipment allocation standards. Methods According to the national guidance on medical equipment allocation in township hospitals, a questionnaire was designed, which covered the laboratory equipments the hospital possessed, the utility and maintenance of these equipments. A survey was carried out in five community health service centers, six central township hospitals and three general township hospitals in Kunshan city from August 2011 to February 2012. Results Community health service centers and township hospitals in Kunshan were all equipped with automatic biochemical analyzer, automatic blood cell analyzer, third group of blood cell analyzer, coagulation analyzer, electrolyte analyzer and urine analyzer. Community health service centers were not equipped with blood gas analyzer, rheometer, enzyme label machines, washing machine and urinary sediment analyzer; while township hospitals were not equipped with rheometer. Among the 30 sets of laboratory equipments pos- sessed by community health service centers, 23 sets were running well, 5 were still working with some flaws, and 2 were scrapped; while among the 90 sets of laboratory equipments possessed by township hospitals 74 were running well, 15 were still working with some flaws, and 1 were scrapped; there was no statistically significant difference （ 2 = 2. 849, P 〉 0.05 ）. Incommunity health service centers, 27 sets were maintained by suppliers with maintenance fee and 1 was maintained by supplier without any maintenance fee; while in township hospitals, the numbers were 87 and 2, respectively （2 =0. 149, P 〉0.05）.
