不同证型子宫内膜异位症与血清实验室指标相关性分析

摘　　要：目的探讨不同证型子宫内膜异位症与血清实验室指标的相关性。方法选择天津市南开医院妇产科2012年9月—2013年10月因卵巢子宫内膜异位症行经腹腔镜或传统开腹手术的患者137例,分辨中医证型,检测血清实验室指标(包括血常规、生化、肿瘤标记物),然后对不同证型与血清实验室指标的相关性进行多因素Logistic回归分析。结果 137例患者中气滞血瘀型59例(43.1%),肾虚血瘀型42例(30.7%),寒凝血瘀型15例(10.9%),湿热血瘀型10例(7.3%),痰瘀互结型5例(3.6%),其他6例(4.4%)。多因素Logistic回归分析结果显示,血小板计数(PLT)、磷(P)、糖类抗原125(CA125)与气滞血瘀型子宫内膜异位症有回归关系;P、CA125与肾虚血瘀型和寒凝血瘀型子宫内膜异位症有回归关系。结论气滞血瘀型、肾虚血瘀型、寒凝血瘀型子宫内膜异位症均与P、CA125有关,气滞血瘀型同时与PLT相关。Objective To investigate the correlation between various laboratory test parameters and subtypes of endo- metriosis classified according to the traditional Chinese medicine theory. Methods A total of 137 patients with ovarian endometri- osis undergoing laparoscopy or conventional laparotomy in the Department of Obstetrics and Gynecology of Nankai Hospital in Tianjin between September, 2012 and October, 2013 were enrolled. According to the traditional Chinese medicine theory, endo- metriosis was classified as： a Qi stagnation （n =59） subtype, a kidney vacuity -related blood stagnation subtype （n =42）, a congealing cold - related blood stagnation subtype （ n = 15 ）, a sweaty heat - related blood stagnation subtype （ n = 10）, a phlegm - blood stasis combination subtype （ n = 5 ）, and a unclassified subtype （ n = 6）, respectively. Blood samples were col- lected from all subjects. Laboratory tests including routine blood and biochemical tests, and tumor marker analysis were per- formed. The correlation between different syndrome subtypes of endometriosis and various laboratory test parameters was analyzed by multivariate logistic regression analysis. Results Platelet count and serum levels of phosphorous and cancer antigen CA^25 were correlated with the Qi stagnation subtype of endometriosis. Serum levels of phosphorus and cancer CA^25 were correlated with the kidney vacuity - related blood stagnation subtype of endometriosis. Conclusion Different laboratory test parameters may help de- fine symptom subtypes of endometriosis and thus have significant clinical implications.
