芪参益气滴丸对Ameroid缩窄环致心功能不全小型猪心功能及心室重构影响

摘　　要：目的观察芪参益气滴丸对Ameroid缩窄环致心功能不全小型猪心功能及心室重构的影响。方法巴马小型猪22只,采用左冠状动脉前降支放置缩窄环的方式制备小型猪慢性心肌缺血心功能不全模型,随机分为模型组、地高辛组、芪参益气滴丸组,每组6只,另制备假手术动物4只。于术后第4周开始口服给药,地高辛组每日1次0.012 5 mg/kg,芪参益气滴丸组每日3次,每次0.025 g/kg,持续8周。于给药前和给药后2、4、8周末超声心动检测各组动物心功能,于给药后第8周末取材进行心肌组织病理观察。结果超声心动结果显示,给药前,模型动物左室射血分数(EF)显著下降(P<0.01);给药后2周,地高辛组动物左室舒张末容积(LVDV)、左室收缩末容积(LVSV)、EF显著改善(P<0.05);给药后4周芪参益气滴丸组动物LVDV、LVSV、EF显著改善(P<0.05或P<0.01)。HE染色显示,模型动物左心室心肌出现细胞结构异常和大量炎细胞浸润,芪参益气滴丸组心肌细胞排列紊乱程度减轻,炎细胞浸润程度轻于模型组。Masson染色显示,模型动物心肌胶原呈现弥漫性分布,芪参益气滴丸组胶原呈树突状分布,胶原含量少于模型组。结论芪参益气滴丸能够显著改善Ameroid缩窄环致心肌缺血心功能不全小型猪心功能并抑制心室重构。Objective To observe the influences of Qishen Yiqi Gutta Pills on heart function and ventricular remodeling in minipigs with heart failure induced by Ameroid constrictor. Methods The model of chronic myocardial isehemie heart failure was established in Bama minipigs （ n = 22 ） through putting Ameroid constrictor in left anterior descending coronary artery, and then divided into model group, digoxin group and Qishen Yiqi Gutta Pills group （ Qishen group, each n = 6） and other 4 included into sham - operation group. On the 4th week after the operation, digoxin group was given digoxin once a day （0. 0125 mg/kg once） and Qishen group was given Qishen Yiqi Gutta Pills 3 times a day （0.025 g/kg once） for 8 weeks. The heart function was detected by using echocardiogram in all groups before medication and on the 2nd week, 4th week and 8th week after medication. The pathological changes of myocardial tissue were observed on the 8th week after medication. Results The results of echocardiogramshowed that LVEF decreased significantly （ P 〈 0.01 ） in model group betore medlcaUon. LV L, V, L v ~ v and EF% were significantly improved （P 〈 0.05） in digoxin group 2 weeks after medication, and those were significantly improved （ P 〈 0.05 or P 〈 0.01 ） in Qishen group 4 weeks after medication. HE staining showed that abnormal structure of myocardial cells and a lot of inflammatory cell infiltration in model group, which were relieved in Qishen group. Masson staining showed that diffuse distribution of myocardial collagen in model group, and dendritic distribution in Qishen group. The content of myocardial collagen was less in Qishen group than that in model group. Conclusion Qishen Yiqi Gutta Pills can significantly improve heart function and inhibit ventricular remodeling in minipigs with myocardial ischemic heart failure induced by Ameroid constrictor.
