结核感染T细胞斑点试验结合MRI诊断足踝部结核有效性分析

摘　　要：目的评价结核感染T细胞斑点试验(T-SPOT.TB)结合MRI诊断足踝部结核的有效性。方法180例疑似足踝部结核患者接受体检、MRI检查、T-SPOT.TB检测及组织病理检查,并以组织病理检查阳性作为确诊的金标准。结果 180例疑似患者中21例确诊为足踝部结核。T-SPOT.TB检测诊断足踝部结核的敏感性和特异性分别为85.71%和96.86%,MRI检查分别为76.20%和94.34%,T-SPOT.TB联合MRI检查分别为71.43%和99.37%。结论 T-SPOT.TB结合MRI检查是一种可有效地诊断足踝部结核的方法。Objective To verify the availability of T-SPOT. TB test combined with MRI in the diagnosis of tuberculosis of foot and ankle. Methods One hundred eighty patients suspected of tuberculosis of foot and ankle underwent physical examination, MRI examination and T SPOT. TB test. Meanwhile tissue samples of all patients were sent for histopathologic examination. Results Faking histopathology as the gold standard, 21 cases of the 180 suspected patients were confirmed as tuberculosis of foot and ankle. T SPOT. TB test has a sensitivity of 85. 71% and a specificity of 96. 86%. Meanwhile MR1 has a sensitivity of 76. 20% and a specificity of 94.34%. T SPOT. TB combined with MRI has a sensitivity of 71.43% and a specificity of 99.37%. Conclusions T SPOT. TB combined with MRI is an effective method for diagnosing the tuberculosis of foot and ankle.
