宫腔镜联合B超在取出嵌顿宫内节育器中的应用

摘　　要：目的探讨宫腔镜联合B超在取出嵌顿宫内节育器(IUD)中的临床应用。方法分析2008年1月至2013年5月首都医科大学燕京医学院附属良乡医院收治的37例IUD嵌顿患者的临床资料。结果 37例患者中IUD类型:T形4例,圆形13例,伽马环8例,吉妮环2例,V型10例。外院及门诊取环失败8例。绝经前12例,绝经后25例。34例宫腔镜联合B超取环成功。3例残环完全嵌顿子宫肌层,患者无症状,随访观察。结论宫腔镜联合B超用于取出嵌顿宫内节育器安全有效。Objective To explore the clinical application of hysteroscopy combined B type ultrasonography in taking out the inlaid intrauterine device. Methods The clinical data of 37 cases with intrauterine devices in this hospital during January 2008 to May 2013 were retrospectively reviewed and analyzed. Results The types of IUD in 37 cases were as follows： T shaped plastic frames in 4 cases, round shaped frames in 13 cases, Gamma rings in 8 cases, Gyne Fix frameless IUDs in 2 cases and V shaped frames in 10 cases. Eight cases of them were failed in taking out the IUD in other hospitals and outpatient clinic of this hospital. The IUDs of 12 cases were taken out before menopause, and IUDs in 25 of them were taken out after menopause~ The taking out of IUD had been successful in 34 cases with hysteroscopy under the guidance of B type ultrasonography. The residual IUD had been inlaid in the muscular layer of uterus in 3 cases, but without any symptoms during the follow - up period. Conclusion The application of hysterescopy combined B type ultrasonography in taking out the inlaid intrauterine device （IUD） in uterus is effective and safe.
