糖耐量异常患者早期肾损害的临床研究

摘　　要：【目的】探讨糖耐量异常（IGT）患者早期肾功能的损害情况。【方法】选取本院自2011年6月至2012年6月收治的IGT患者96例，依据血压情况将患者分为正常血压组（33例）、临界高血压组（30例）、高血压组（33例），另取健康体检正常者80例为对照组，分别测定患者的尿微量白蛋白排泄率（UAE ）、尿N-乙酰-β-D-氨基葡萄糖苷（NAG）、尿肌酐（Cr）、血清半胱氨酸蛋白酶抑制剂C（Cystatin C）等指标的水平。【结果】IGT患者中各组与正常对照组相比，各指标均显著升高，其差异有统计学意义（P＜0．01，P＜0．05）；病例组之间随着血压升高，各检测指标均明显升高，其差异有统计学意义（P＜0．01，P＜0．05）。【结论】IGT患者早期已出现肾功能损害。应及早通过联合测定尿UAE和NAG等指标并通过给予患者肾保护治疗以预防肾损害。[Objective] To explore the early kidney damage of patients with impaired glucose tolerance（IGT） .[Methods]A total of 96 IGT patients in our hospital from June 2011 to June 2012 were chosen .Based on blood pressure ,all patients were divided into normal blood pressure group （ n = 33） ,critical hypertension group（ n =30） ,high blood pressure group（ n=33） and normal control group（ n=33） .Urine albumin excretion rate（UAE） , urinary n-acetyl-beta-D-amino glucoside（NAG） ,urine creatinine （Cr） ,serum levels of cysteine protease inhibitors C（Cystatin C） and other indicators were determined .[Results]Compared with normal control group ,all indexes in IGT patients were markedly increased ,and there was significant difference between IGT groups and control group（ P 〈0 .01 ,P 〈0 .05） .All indexes in case groups were obviously increased with blood pressure ,and there was significant difference（ P 〈0 .01 ,P 〈0 .05） .[Conclusion] IGT patients have early renal damage .The deter-mination of urine UAE combined with NAG and renal protection treatment should be taken as soon as possible in order to prevent renal damage .
