注射处方不合理用药分析

摘　　要：目的对医院中心药房不合理注射处方进行分析,以为临床合理用药提供参考。方法对医院中心药房2012年6～12月注射剂配剂处方进行回顾性分析,对不合理处方进行统计、分析。结果经审查发现注射剂用药不合理处方156例,其中注射途径不合理9例(5.77%),溶媒及溶媒量选择不合理90例(57.69%),不合理配伍13例(8.33%),用药频次不合理27例(17.31%),用量不合理17例(10.90%)。结论通过临床药师对注射处方的审核,及时发现并纠正不合理用药现象,提高患者用药安全性和有效性,充分体现临床药师的价值。Objective Through carrying on the analysis to the unreasonable injection prescription in the center phar- macy of our hospital, to provide references for clinical rational drug use. Methods The injection prescriptions of the center pharmacy of our hospital from June to December in 2012 were retrospectively analyzed, and the unreasonable prescriptions were to compile statistics and analyzed. Results After review, we found that unreasonable injection prescriptions were156 cases. The injection way is not reasonable accounted for 9 cases（5.77% ） ,solvent and the solvent amount choice is unreasonable in 90 cases （57.69%）, 13 cases of unreasonable compatibility, the use frequency of 27 cases was unreasonable, irrational amount in 17 cases. Conclusion Through the review of clinical pharmacists to injection prescription, we timely discover and correct the phenomenon of irrational use of drugs and improve safety and efficacy in patients with medication ,which fully embodies the value of pharmacists.
