不同止血手术治疗难治性产后出血的临床效果

摘　　要：目的探讨不同止血手术治疗难治性产后出血的临床效果。方法选取本院2009年1月~2012年12月收治的难治性产后出血患者176例,将其随机分为介入治疗组和结扎治疗组,每组88例,介入治疗组采用盆腔动脉栓塞治疗法,结扎治疗组采用盆腔血管结扎治疗法。观察比较两组的术中出血量、总出血量、手术时间及止血成功情况。结果介入治疗组的术中出血量、总出血量少于结扎治疗组,手术时间短于结扎治疗组,两组差异有统计学意义（P〈0.05）;介入治疗组的总止血成功率为100.00%,显著高于结扎治疗组的57.95%,差异有统计学意义（χ2=46.8489,P=0.0000）。结论采用盆腔动脉栓塞治疗难治性产后出血,与采用盆腔血管结扎治疗法比较,具有更好的止血效果,产妇的出血量更少,止血成功率更高,是临床治疗难治性产后出血的理想选择。Objective To explore the clinical efficacy of different operations on treating intractable postpartum hemor- rhage.Methods 176 cases of patients presented with intractable postpartum hemorrhage from January 2009 to December 2012 in our hospital were selected and evenly divided into interventional therapy group and ligation treatment group in random.They were treated by pelvic arterial embolization and pelvic vessel ligation respectively,intraoperative amount of bleeding,total blood loss,operation time,and success of stanch bleeding in both groups was observed and compared.Re- suits In interventional therapy group,the intraoperative amount of bleeding,total blood loss was less,and operation time, was shorter than that in ligation treatment group,with statistical difference （P〈O.05）.The total success rate of stanching bleeding in the interventional therapy group reached 100.00% ,much higher than 57.95% in the ligation treatment group,with statistical difference （X2=46.8489,P=0.0000）.Conclusion In the practice process of treating intractable post- partum hemorrhage in clinic,compared with pelvic vessel ligation,application of pelvic arterial embolization obtains a better hemostatic effect at advantages of lower amount of bleeding in puerpera and higher success rate of stanching bleeding,which is an optimal choice for tackling intractable postpartum hemorrhage in clinic.
