基于病种的基层医疗机构基本医疗服务范围界定探析

摘　　要：目的探讨我国基层医疗机构基本医疗服务范围，为基本医疗服务相关目录的制定提供参考。方法通过对我国东中西部地区6省市541家基层医疗机构关于门急诊和住院服务内容的调研，利用病种综合值框定基本医疗病种。结果2010年城乡基层医疗机构基本医疗服务门急诊病种41个，覆盖人次占门急诊总人次的82％；2010年城乡基层医疗机构基本医疗服务住院病种24个，覆盖人次占住院总人次的82．53％，覆盖费用占住院总费用的79．20％。结论城乡基层医疗机构是保障我国居民基本医疗服务需求的重要卫生服务机构，利用病种综合值框定我国基层医疗机构基本医疗服务病种，能为我国基层医疗机构基本医疗服务范围的确定提供一定参考。Objective To explore the scope of basic medical services at primary health organizations in China and offer references for making corresponding lists of such basic services. Methods The investigation covered 541 primary health organizations in six provinces and cities in China, and the diseases of basic medical services were rounded up with the disease integrated values. 17amflts The survey found 41 common diseases for both outpatient/emergency departments at primary health organizations in 2010, covering 82% of the total outpatients. And there were 24 common diseases in inpatient environment at primary health organizations in 2010, covering 82. 53% of the total inpatients, and accounting for 79. 20% of the total inpatient costs. Ceadusion Primary health institutions are critical medical service providers which guarantee basic medical care services in rural and urban areas in China. The integrated value for single diseases may be used to identify common diseases within basic medical services. This will provide references for rounding up the scope of basle medical services of such organizations.
