广西地区医院消毒供应中心清洗消毒灭菌质量控制的调查研究

摘　　要：目的分析医院消毒供应中心质量技术管理存在的薄弱环节和问题,并提出管理对策。方法对广西158所医院消毒供应中心进行问卷调查,对其中47所进行现场核查。结果共回收141份有效问卷。2所医院的清洗、消毒和灭菌业务委托它院处理。省、市、县级医院消毒供应中心3种清洗操作方法的使用率差异有统计学意义;141所医院消毒供应中心清洗消毒程序有效性的监测方式为打印运行记录、确认符合厂家使用说明和观察运行程序时间、温度,比例分别为53.9%(76/141)、73.1%(103/141)、84.4%(119/141);136所医院消毒供应中心使用预真空压力蒸汽灭菌器,86.0%(117/136)每日空载进行B-D试验,97.1%(132/136)每周行生物监测,75.7%(103/136)对植入物灭菌每炉次生物监测。现场核查44所医院消毒供应中心的医用热封机使用情况,88.6%(39/44)每日使用前检测参数和闭合完好性,3所配置非医用热封机。结论我区医院消毒供应中心需完善设备配置,优选清洗技术操作和监测方法,将过程检测程序化、效果监测规范化,推行危害分析与关键控制点质量管理,预防和控制医院感染的发生。Objective To investigate the situation of quality control of cleaning,disinfection and sterilization in the Central Sterile Supply Department（CSSD）,in order to identify the existing problems and propose management strategies. Methods A questionnaire survey was conducted in 158 CSSDs in Guangxi Zhuang Autonomous Region and a field survey was conducted in 47 hospitals. Results Totally 143 CSSDs responded to the survey,among which the medical devices in two hospitals were sent to other hospital＇s CSSDs for disinfection and sterilization. The cleaning methods were different in CSSDs among different level hospitals. In 141 CSSDs,53.9%（76/141） verified the effectiveness of cleaning and disinfection procedures by printing the running record,73.1%（103/141） by the manufacturers＇ instructions,and 84.4%（119/141） by observing the time and temperatures. In 136 CSSDs with pressure steam sterilizer,86%（117/136） did B-D test daily at the state of non-load before sterilization,97.1%（132/ 136） performed biological monitoring weekly,75.7%（103/136） performed biological monitoring of implants steriliza- tion of each furnace. In the field survey,88.6% of 44 CSSDs with heat-sealing machine,checked the parameters of heat-sealing machine before daily use. Conclusions CSSD staff should improve the equipments,optimize cleaning and monitoring methods,carry out routine process verification and standardize outcome verification and implement risk analysis and quality management on the key points to prevent and control nosocomial infection.
