小柴胡汤联合多靶点疗法治疗难治性肾病综合征临床研究

摘　　要：目的:探讨小柴胡汤联合多靶点疗法治疗难治性肾病综合征(RNS)的临床疗效及安全性。方法:将134例RNS患者随机分为对照组及观察组,对照组给予多靶点疗法,即小剂量尿激酶、贝那普利、缬沙坦、霉酚酸酯及激素等,观察组在此基础上联合小柴胡汤,并随证加减,疗程结束后比较两组的疗效及生化指标。结果:对照组脱落10例,观察组9例,共115例完成研究,对照组总有效率68.4%,观察组84.5%,两组临床疗效比较差异有统计学意义(P<0.05)。治疗后两组及同组治疗前后总胆固醇、白蛋白、24 h尿蛋白水平比较,差异有统计学意义(P<0.05),而Ccr水平两组治疗后及同组治疗前后比较,差异无统计学意义(P>0.05)。对照组不良反应发生率为23.8%,观察组9.8%,观察组不良反应发生率低于对照组(P<0.05)。结论:小柴胡汤联合多靶点疗法治疗RNS,能够提高疗效,并可降低不良反应的发生。ABSTRACT Objective ： Objectives To explore clinical efficacy and safety of Xiaochaihu Decoction combined with multi targeted therapy in the treatment of refractory nephrotic syndrome （RNS）. Methods ：134 Patients with RNS were enrolled and were ran domly divided into observation group （n = 67） and control group （n = 67）. The patients in control group were treated with multi tar geted therapy, including small dose urokinase, Benazepril, valsartan, mycophenolate mofetil and hormone, while observation group were treated with Xiaochaihu Decoction combined with multi targeted therapy and added or subtracted with symptom. Test being fin ished the therapeutic effect and biochemical indicators were compared. Results：Control group 10 cases fell out and the observation 9 cases. A total of 115 patients completed the study. The total effectiveness of control group was 68.4% and that of observation was 84.5%. There was significant difference between two groups （P 〈 0.05 ）. About he level of total cholesterol, albumin and 24 hour u rinary protein there were significant differences after treatment between two groups and between before and after treatment in same group （P 〈0.05）. So were not the levels of creatinine clearance rate（P 〉0.05）. The incidence of adverse reaction of control group was 23.8% and observation group was 9.8%. There was significant difference too （ P 〈 0.05 ）. Conclusion： Xiaochaihu Decoction combined with multi targeted therapy can improve curative effect of RNS and reduce adverse reaction.
