雌三醇软膏联合宁泌泰胶囊治疗女性复发性尿路感染临床疗效观察

摘　　要：目的该研究从症状评分、细胞免疫功能及复发率探讨雌三醇阴道软膏联合宁泌泰胶囊治疗绝经后妇女复发性尿路感染的临床疗效。方法选取自然绝经或手术绝经6个月以上,经尿培养确诊为反复尿路感染且排除特异性泌尿系感染,采用随机分组的方法分为环丙沙星组(A组)43例,环丙沙星+雌三醇阴道软膏组(B组)51例,环丙沙星+宁泌泰+阴道雌三醇软膏组(C组)46例。结果三组患者经治后的症状均得到了改善,C组较A、B组改善明显(P<0.05);在C组经治疗后CD+4T细胞明显上升,CD+8T细胞明显下降,尿SIgA升高,与治疗前比较有显著性差异(P<0.05),且较A、B组具有显著差异(P<0.05)。A组复发率为69.8%,B组复发率为13.7%,C组复发率为6.5%,C组较A、B组复发率显著降低(P<0.05)。结论雌三醇阴道软膏有效延长绝经后妇女复发性尿路感染的复发间隔时间,宁泌泰胶囊具有改善患者的临床症状,并可提高患者细胞免疫功能,两者均可降低复发率,临床可联合使用。Objective To study the clinical efficacy of estriol vaginal cream versus suppository combined with Ningmitai capsule to treat recurrent urinary tract infection in postmenopausal women. Methods A number of 140 women with natural menopause or operation menopause for more than 6 months were selected, whose urine culture was diagnosed as recurrent uri- nary tract infection with no specific urinary infection. The patients were randomly divided into 3 groups, ciprofloxacin group （group A, n=43）, ciprofloxacin 4- estriol vaginal cream versus suppository group （group B, n =51）, and ciprofloxacin 4- Ningmitai 4- estriol vaginal cream versus suppository group （group C, n=46）. Results The symptoms were improved after therapy, and the group C improved much better than group A and B, with significant difference among the 3 groups （P〈 0.05）. In group C, the CD＋ T and urine SIgA increased, and CD＋T decreased （P〈0.05）, and there were significant differ- ences compared with group A and B （P〈0.05）. The recurrence rate was 69.8 % in group A, 13.7 % in group B, and 6.5 % in group C, with significant differences （P〈0.05）. Conclusions The estriol vaginal cream versus suppository effectively ex- tends the recurrence interval of recurrent urinary tract infection. Ningmitai capsule can improve clinical symptoms and the cel- lular immune function of patients. They can be combined for use in clinical practice.
