快通道外科理念在老年髋关节置换术护理中的应用

摘　　要：目的 尝试将快通道外科（fast track surgery,FTS）理念应用于老年髋关节置换围手术期中护理的安全性和可行性.方法 选择60例在骨科实施髋关节置换手术治疗的老年股骨颈骨折患者,其中2010年1月至12月期间手术的30例患者为对照组,按照传统的方法进行治疗和护理;2011年1月至12月期间手术的30例为观察组,围手术期进行FTS治疗和护理,术后按照3、6、9、12 m四个时段进行随访,由专人运用Harris评分表进行评估,比较两组患者术后2w并发症发生率和髋关节功能康复进程.结果 与对照组比较,观察组术后2w并发症的发生率显著减少,髋关节功能康复进程加快,两组比较差异有统计学意义（P＜0.05）.结论 将FTS理念应用于老年髋关节置换手术患者的护理是安全可行的,减少了并发症的发生,加速了患者康复进程.Objective To analyze the security and feasibility of fast track surgery （FTS） concept applied to the elderly hip replacement perioperative period nursing.Methods Using controlled clinical studies,we selected 60 patients in our hospital orthopaedic implement hip replacement surgery in elderly patients with femoral neck fracture,including nursing during the period from January to December 2010 30 patients in the control group （traditional group）,according to the traditional methods for treatment and care; operation during the period from January to December 2011 30 cases as the observation group,perioperative fast channel treatment and nursing care,postoperative according to 3 m,6 m,9 m and 12 m of four period follow-up,applied to evaluate Harris scale,we compared two groups of patients with postoperative complication rate of 2 w and the hip joint function rehabilitation process.Results Compared with the control group,observation group significantly reduced the incidence of postoperative complications of 2 w,the hip joint function rehabilitation process was accelerated,the comparative difference was statistically significant （P ＜ 0.05）.Conclusion Applying the idea of FTS in elderly patients with hip replacement surgery nursing is safe and feasible,and it can reduce the complications,and accelerate the process.
