摘  要:目的研究不同经后龄(PMA)早产儿振幅整合脑电图(aEEG)的变化规律。方法选取2012年1-10月中国人民解放军白求恩国际和平医院新生儿病区108例不同孕周的早产儿,共分为28~36周9组,每组12例。采用国产脑功能监护仪CFM3000)分别在生后12h内连续监测其aEEG波谱带的变化,分析不同PMA早产儿aEEG背景连续性、睡眠一觉醒周期(SWC)、下边界电压振幅和窄带宽度,探讨其变化规律。结果108例早产儿aEEG波形连续性显示:(1)随着PMA增高,连续电压所占比例逐渐升高,不同PMA早产儿连续电压出现的比例分别为28周(0例)、29周[1例(8.33%)]、30周[3例(25%)]、31周[3例(25%)]、32周[7例(58.33%)]、33周[11例(91.67%)]、34周:12例(100.0%)]、35周[12例(100.0%)]、36周[12例(100.0%)]。各组间比较差异有统计学意义(χ2=24.003,P〈0.01)。(2)各孕周的SWC出现率分别为:28周(0例)、29周(0例)、30周[1例(8.33%)]、31周[1例(8.33%)]、32周[3例(25%)]、33周[7例(58.33%)]、34周[11例(91.67%)]、35周[12例(100.0%)]、36周[12例(100.0%)]。各组间比较差异有统计学意义(χ2=36.742,P〈0.01)。30~32周偶有出现SWC,但多数为不成熟周期,33周开始多数出现周期,但周期仍欠成熟,34周以上均有较成熟的SWC。(3)随PMA增长,脑电波下边界逐步增高,下边界从28周后的(4.29±1.08)μV逐步上升到36周的(9.68±1.63)μV(F=56.943,P〈0.01)。(4)波谱带宽度逐步变窄,从28周的(45.4±4.14)μV下降到36周的(8.76±2.71)μV(F=147.936,P〈0.01)。结论早产儿脑发育过程中aEEG有其自身的变化规律,对判断脑发育成熟度有一定的参考价值。Objective To study amplitude integrated EEG (aEEG) variation of different postmenopausal age (PMA) premature infants. Methods A total of 108 premature infants of different gestational age including 58 males and 50 females were recruited from January 2012 to October 2012, using domestic CFM3000 brain function monitors. All the premature infants received continuous brain function monitoring for 12 hours in 12 hours after birth. The continuity, sleep-wake cycle, low bound voltage and bandwidth of the narrow band were analyzed. Results ( 1 ) The percentage of continuity voltage in differen postmenstrual age infants was 28-week (0),29-week (8.33%), 30-week (25%), 31-week (25%), 32-week (58.33%), 33-week (91.67%), 34-week (100%), 35-week (100%), 36-week (100%) (χ2 = 24.003, P 〈 0.01 ). (2)The percentage of mature sleep-wake cycle increased from 0% in 28-week premature infants to 100% in 36-week premature infants (χ2 =36.742, P 〈 0.01 ). (3)Low bound voltage increased with gestational age, from (4.29± 1.08)μV(28-week) to (9.68± 1.63)μV (36-week) (F=56.943,P〈 0.01 ). (4)Bandwidth of the narrow band decreased gradually with gestational age, from (45.4± 4.14)IxV(28-week) to (8.76± 2.71 )μV (36-week) (F=147.936, P 〈 0.01 ). Conclusion Brain development in premature infants during the aEEG has its own variation;the aEEG shows some specific regulation in different PMA preterm infants. It can be used for judging the degree of brain maturation.
