经鼻纤支镜引导下气管插管加吸痰术在抢救重症呼吸衰竭患者中的应用

摘　　要：目的：分析经鼻纤支镜引导下气管插管加吸痰术在抢救重症呼吸衰竭患者中的应用效果。方法选取我院2010年1月至2012年1月收治的80例重症呼吸衰竭患者，对患者行经鼻纤支镜引导下气管插管加吸痰术抢救，分析临床疗效。结果经鼻纤支镜引导仅需要3～6 min，该时间段患者心率、血氧饱和度均无明显变化（P＞0．05），并且操作对患者组织损伤极小，无出血案例。经鼻纤支镜引导下气管插管加吸痰术抢救后，患者气道压力、血氧饱和度显著高于手术前（P ＜0．05），且心率无明显变化。结论经鼻纤支镜引导下气管插管加吸痰术安全可靠，对患者损伤极小，无相关并发症，可极大地降低患者呼吸气道压力，改善患者肺部通气功能、痰液引流情况。Objective To analyze the application efficacy of trachea cannula guided by bronchoscope and suctioningin rescuing patients suffered from severe respiratory failure .Methods 80 cases of severe respiratory failure patients, who received treatment in our hospital during January 2010 and January 2012, were included in this study.They were rescued by using the method of trachea cannula guided by bronchoscope and suctioning , and analyze it＆#39;s clinical efficacy .Results The time spent on trachea cannula under bronchoscope guidance only need 3-6min.And heart rate, oxygen saturation did not change significantly （P＆gt;0.05） during this period, and none surgical bleeding occurred .Airway pressure and oxygen saturation were significantly higher than those of pre-operation（P＆lt;0.05）, and heart rate did not changed .Conclusions Trachea cannula guided by bronchoscope and suctioning was safe , without serve injury and complications , could reduce respiratory airway pressure and improve pulmonary ventilationfunction and drainage sputum situation .
