老年股骨转子间骨折PFNA固定与半髋置换手术治疗对比

摘　　要：目的观察PFNA固定与半髋置换手术治疗老年股骨转子间骨折的疗效。方法观察组24例,行半髋置换手术治疗,对照组18例,行PFNA固定治疗。术后比较两组患者治疗疗效差异及手术具体情况。结果观察组的手术时间、术后卧床时间及术后有限负重时间均远小于对照组,差异具有显著统计学意义(P<0.05)。半髋置换手术治疗的优良率为100.0%,远大于PFNA固定治疗72.2%,差异具有显著统计学意义(χ2=14.966,P<0.05)。结论半髋置换手术治疗老年股骨转子间骨折疗效显著,患者依从性好,病愈时间短,并发症容易控制,值得临床推广。Objective To observe the effect of PFNA fixation and hemiarthroplasty of hip operation in treatment of intertrochanteric fracture. Methods 24 cases in observation group, semi hip replacement operation treatment, 18 cases in the control group, PFNA fixation. Treatment differences and operation conditions were compared between two groups of patients after operation. Results The operation time of the observation group, postoperative recovery time and postoperative limited load time was far less than that of the control group, there was a statistically significant difference （P〈0.05）. The excellent and good rate of treatment of semi hip replacement operation was 100%, much higher than that of PFNA fixation in the treatment of 72.2%, there was a statistically significant difference （χ2=14.966, P〈0.05）. Conclusion Semi hip replacement operation significant efficacy in the treatment of intertrochanteric fracture of femur in the elderly, patients with good compliance, recovery time is short, easy to control the complications, worthy of clinical application.
