摘  要:目的:探讨经寰枢椎椎弓根螺钉固定、融合治疗寰枢关节失稳的临床可行性及其疗效。方法2006年11月-2012年9月收治的寰枢关节失稳或脱位患者病例24例。所有患者均采用经椎弓根螺钉固定、融合治疗方法,对患者进行随访。结果手术中无脊髓和椎动脉损伤的病例。寰枢椎完全复位15例,不完全复位9例。随访结果,寰枢关节复位较好,均获得骨性融合,融合时间为3-11个月。结论经椎弓根固定能为失稳或脱位的寰枢关节提供足够的力学稳定性,具有操作简便、稳定性重建好,手术并发症少等优点,在技术上操作上具有可推广性。Objective To investigate the clinical feasibility and efficacy of atlantoaxial pedicle screw fixation, fusion for atlantoaxial joint failure. Methods From 2006 November to 2012 September in our hospital, atlantoaxial instability or dislocation in 24 cases. All patients were treated by pedicle screw fixation, fusion methods of treatment, the patients were followed. Results The operation without damage to the spinal cord and vertebral artery. Atlantoaxial reposition completely in 15 cases, 9 cases of incomplete reduction. The follow-up results, atlantoaxial restoration, bony fusion, the fusion time was 3-11 months. Conclusion Transpedicular fixation can provide enough mechanical stability to instability or dislocation of the atlantoaxial joint, and has the advantages of simple operation, good stability of reconstruction, the advantages of fewer complications in operation, technical operation with extension.
