摘 要:目的 探讨切口痛-瑞芬太尼痛觉过敏大鼠脊髓和背根神经节δ阿片受体表达水平的变化.方法 取尾静脉置管成功的成年雄性SD大鼠32只,体重260~ 280 g,采用随机数字表法将其分为4组(n=8):对照组(C组)静脉输注生理盐水0.1 ml·kg-1 ·min-1 60 min;切口痛组(Ⅰ组)静脉输注生理盐水0.1 ml·kg-1 ·min-1 60 min,于输注即刻建立切口痛模型;瑞芬太尼组(R组)静脉输注瑞芬太尼1.0 μg· kg-1·min-1 60 min;瑞芬太尼+切口痛组(RI组)静脉输注瑞芬太尼1.0 μg·kg-1 ·min-1 60min,于输注即刻建立切口痛模型.于静脉输注生理盐水或瑞芬太尼前24 h、输注后2、6、24和48 h(T0-4)时,在室温(18~22℃)安静环境下测定机械刺激缩足阈(PWT)和热刺激缩足潜伏期(PWL).行为学测试结束后处死大鼠,取脊髓L4-6节段和背根神经节,采用Western blot法测定脊髓和背根神经节总蛋白及膜蛋白δ阿片受体的表达,并计算膜蛋白与总蛋白δ阿片受体表达的比值(m/t比值).结果 与C组比较,Ⅰ组、R组和RI组T1-4时PWT降低,PWL缩短,总蛋白和膜蛋白δ阿片受体表达上调,m/t比值增加(P<0.05或0.01);与Ⅰ组比较,R组T1-4时PWT、PWL、总蛋白和膜蛋白δ阿片受体表达和m/t比值差异无统计学意义(P>0.05),RI组T1-4时PWT降低,PWL缩短,总蛋白和膜蛋白δ阿片受体表达上调,m/t比值增加(P<0.05或0.01);与R组比较,RI组T1-4时PWT降低,PWL缩短,总蛋白和膜蛋白δ阿片受体表达上调,m/t比值增加(P<0.05或0.01).结论 大鼠切口痛-瑞芬太尼痛觉过敏的形成可能与脊髓和背根神经节δ阿片受体表达上调和向细胞膜上转运增强有关.Objective To investigate the changes in δ opioid receptor expression in the spinal cord and dorsal root ganglias in rats with hyperalgesia induced by incisional pain (IP)-remifentanil.Methods Thirty-two adult male Sprague-Dawley rats in which the catheter was successfully placed in the caudal vein were randomly divided into 4 groups (n =8 each):control gourp (group C),IP group (group I),remifentanil group (group R),and remifentanil + IP group (group RI).Normal saline 0.1 ml· kg-1 · min-1 was infused intravenously for 60 min in group C.In group I,normal saline 0.1 ml · kg-1 · min-1 was infused intravenously for 60 min,and IP was induced immediately after onset of infusion.In group R,remifentanil 1.0 μg· kg-1 · min-1 was infused intravenously for 60 min.In group RI,remifentanil 1.0μg· kg-1 · min-1 was infused intravenously for 60 min and IP was induced immediately after onset of infusion.At 24 h before infusion and 2,6,24 and 48 h after infusion (T0-4),paw withdrawal threshold (PWT) and paw withdrawal latency (PWL) were measured at room temperature (18-22 ℃).The rats were sacrificed after the end of the behavioral test and L4-6 segments of the spinal cord and dorsal root ganglias were removed for determination of the expression of total protein and membrane protein δ opioid receptor by Western blot.The ratio of δ opioid receptors in the membrane protein to those in the total protein (m/t ratio) was calculated.Results Compared with group C,PWT was significantly decreased and PWL was shortenedat T1-4,and the expression of total prtein and membrance protein δ opioid receptor was up-regulated and m/t ratio was increased in I,R and RI groups (P 〈 0.05 or 0.01).Compared with group I,no significant change was found in PWT and PWL at T1-4,and the expression of total prtein and membrance protein δ opioid receptor and m/ t ratio in R group (P 〉 0.05),and PWT was significantly decreased and PWL was shortened at T1-4 and the expression of total prtein
