摘　　要：目的调查分析医院住院患者在《2011年全国抗菌药物临床应用专项整治活动方案》执行前后特殊使用类抗菌药物的使用情况,评价其应用的合理性,旨在探讨临床用药监管体系。方法采用回顾性调查分析方法,调查住院患者在2011年4-6月《2011年全国抗菌药物临床应用专项整治活动方案》执行前766例为对照组和执行后2011年9月-2011年11月干预组225例应用特殊使用类抗菌药物的资料,收集该类抗菌药物的使用信息,根据合理性评价指标,评估特殊使用类抗菌药物临床应用,应用Excel 2007软件进行数据录入和统计处理。结果住院患者在《2011年全国抗菌药物临床应用专项整治活动方案》执行前对照组特殊使用类抗菌药物使用率为4.07%、使用强度为3.90、病原菌送检率为45.95%;执行后的干预组分别为1.28%、2.28、60.44%,执行后医院特殊使用类抗菌药物的使用情况趋于合理。结论医院实施了一系列的措施,完善特殊使用类抗菌药物的管理制度,提高用药合理性,保障了医疗安全。OBJECTIVE To investigate and analyze the situation before and after the application of antibiotics of ＂Special use＂ referenced by National Special Renovate Activity for Clinical Use of Antibiotics 2011, and assess its reasonableness, so as to discuss the medicine supervision system. METHODS A retrospective study was conduc- ted, the 766 cases before intervention from Sept. to Nov. 2011 were regarded as control group, and the 225 cases after intervention from Apr. to Jun. 2011 were regarded as intervention group, and their data were studied. The usage information of the group of ＂special-used＂ antibiotics were collected, based on rationality evaluation index, the clinical application of antibiotics was evaluated. Excel 2007 was used for inputting data and statistical analysis. RESULTS In control group, the frequency of the antibiotics use was 4. 07%, the antibacterial use density was 3.90, detection rate of pathogenic bacteria was 45.95%. Intervention group was 1.28%, 2.28, 60.44%. The usage of ＂special-used＂ antibiotics tended to be reasonable. CONCLUSION Our hospital implement a series of measures to perfect the management of ＂special-used＂ antibiotics, and promote the rational application of antibac- terials to ensure the safety of medision.
