铥激光剥橘式前列腺切除术治疗大体积良性前列腺增生症的临床观察

摘　　要：目的评价经尿道铥激光剥橘式前列腺切除术治疗大体积(前列腺重量>100g)良性前列腺增生症(BPH)的有效性和安全性。方法回顾性分析2012年6月-2013年11月间37例行经尿道铥激光剥橘式前列腺切除术治疗的大体积(前列腺重量>100g)BPH患者的围术期资料,分析围术期并发症的发生情况和手术效果。结果 37例患者均顺利完成手术,手术时间为(62.2±18.1)min,术中出血量为(37.6±14.6)mL,术后膀胱冲洗时间为(21.6±8.7)h,术后导尿管留置时间为(5.5±1.7)d,术后住院时间为(6.2±1.3)d。术中无1例患者需输血,术后无电切综合征发生,无死亡病例。术后1、4周和3个月患者的血红蛋白水平与术前比较,差异均无统计学意义(P值均>0.05)。术后1、4周和3个月,患者的国际前列腺症状评分和生活质量评分均显著低于术前(P值均<0.05),而最大尿流率均显著高于术前(P值均<0.05)。结论经尿道铥激光剥橘式前列腺切除术治疗大体积(前列腺重量>100g)BPH有效且安全。Objective To evaluate the safety and efficacy of thulium laser resection of prostate-tangerine technique for treatment of benign prostatic hyperplasia （prostate weighting more than 100 g）. Methods Thirtyseven patients with prostatic hyperplasia （prostate weighting more than 100 g） were treated by thulium laser resection of prostate-tangerine technique between June 2012 and November 2013 in our hospital. The incidence of postoperative complications and surgical outcome were retrospectively analyzed in the study. Results Surgical operations were successfully performed in all the 37 patients. The average operation time was （62.2±18.1） min and blood loss was （37.6±14.6） mL. Postoperative bladder washout and indwelling catheter lasted for （21.6±8.7） h and （5.5±1.7） d, respectively. No transurethral resection prostate （TURP） syndrome, blood transfusion or death occurred. There were no significant differences between preoperative hemoglobin level or postoperative ones （one week, four weeks and three months） （all P〉0. 05）. Compared with the preoperative values, international prostate symptom score （IPSS） and quality of life （QOL） were significantly decreased and the maximum urinary flow （Qmax） was significantly increased one week, four weeks and three months postoperatively. Conclusion Thulium laser resection of prostate-tangerine technique is safe and effective for a large gland benign prostatic hyperplasia.
