240例骨科围术期活血化瘀类中药注射剂应用合理性分析

摘　　要：目的:调查骨科围术期活血化瘀类中药注射剂临床使用现状,为临床合理用药提供参考。方法:随机选取2011—2013年某院骨科手术患者240例,其中使用瓜蒌皮注射液、大株红景天注射液、注射用血栓通、注射用血塞通、注射用红花黄色素、疏血通注射液患者各40例,对其适应证、给药时间、疗程、用法与用量、溶剂、联合用药、不良反应等情况进行分析。结果:无指征用药53例,占22.08%;平均疗程为(9.29±5.24)d;2种及以上药物联用54例,占22.50%;用药剂量不合理33例,占13.75%;溶剂选择错误率为63.75%。结论:骨科围术期活血化瘀类中药注射剂使用存在无指征用药、选药不当、溶剂选择错误、疗程偏长、剂量偏小等问题。OBJECTIVE：To investigate the clinical use of traditional medicine injections for invigorating blood circulation and eliminating stasis so as to provide references for clinical rational use of drugs. METHODS： A total of 240 orthopedic patients in a hospital between 2011 and 2013 were randomly assigned to receive one of the following drugs： Pericarpium Trichosanthis Injection, Rhodiola kirilowii （Regel）Rege Injection, Xueshuantong for Injection, Xuesaitong for Injection, Safflower Yellow for Injection or Shuxuetong Injection, of 40 each. The indications, time and duration of administration, dosage and administration, solvents, drug combinations and adverse effects were analyzed. RESULTS： 53 patients（22.08% ）used antibiotics without indications; the patients used antibiotics for an average of （9.29 ＋5.24）days; 54（22.50% ）used at least two antibiotics concurrently; 33 （13.75%）involved non-rational use of antibiotics, and the error rate in the choice of solvents was 63.75%. CONCLUSION： The perioperative use of traditional medicine injections in orthopedic patients has problems including antibiotic use in the absence of indications, irrational choice of drugs, wrong choice of solvents, prolonged use and low dosage etc.
