肥胖痰湿体质、非痰湿体质与代谢综合征人群胰岛素抵抗相关指数的比较研究

摘　　要：目的:探讨肥胖痰湿体质人群胰岛素抵抗相关指数的变化,揭示痰湿体质代谢紊乱的程度,为通过痰湿体质辨识早期预防代谢综合征提供依据。方法:选取符合诊断标准的肥胖痰湿组、肥胖非痰湿组、代谢综合征(MS)组以及平和体质对照组,检测其空腹血糖(PFG)、胰岛素(INS)水平,计算胰岛素抵抗指数(HOMA-IR)、胰岛素敏感指数(ISI),同时测量体重指数(BMI)、体脂率(F%)、腰围(WC)、血压、血脂等临床和生化指标。结果:除平和质外,MS组、肥胖痰湿组和肥胖非痰湿组BMI、F%、WC比较无差异(P>0.05);肥胖痰湿组与肥胖非痰湿组血压、血甘油三酯、总胆固醇无显著差异(P>0.05);肥胖痰湿组PFG、INS、HOMA-IR与肥胖非痰湿组比较有显著差异(P<0.01或0.05),与MS组差异不明显。结论:胰岛素抵抗可能是痰湿体质潜在的病理机制,也是痰湿体质区别于非痰湿体质的关键特征,并且其发生要早于血脂、血压等临床检验指标的改变。Objective：To investigate the insulin resistance indexes of people with obesity and phlegm-dampness constitu- tion so as to reveal the degree of metabolic disorders in phlegm-dampness constitution. Methods：Four groups of volunteers were chosen in this study：obesity with phlegm-dampness constitution （OPD） ,obesity without phlegm-dampness constitution （ ONPD）, metabolic syndrome group （MS） and the normal balanced constitution group （N）. In each group, we detected thefasting blood-glucose （PFG） and serum insulin to calculate the insulin resistance indexes such as HOMA-IR and ISI. Mean- while,BMI,F% ,WC, blood pressure and blood lipid were measured or detected. Results：As for BMI, F% and WC, there were no significant differences between the OPD, ONPD and MS groups （ P 〉 0.05）. As for blood pressure, serum TG and TC,there were no significant differences between the OPD and ONPD groups （P 〉 0.05）. As for PFG,INS and HOMA-IR, there were significant differences between the OPD and ONPD groups（P 〈 0.01 or P 〈 0.05 ）. The differences between OPD group and MS group were not notable. Conclusion ： Insulin resistance may be the potential pathological mechanism of phlegm- dampness constitution, and it occurs earlier than the change of blood pressure and blood lipid. So we can make a distinction between phlegm-dampness constitution and the other constitutions using insulin resistance indexes.
