RICU手卫生质量持续改进与医院感染管理的相关性研究

摘　　要：目的探讨RICU医务人员手卫生质量持续改进与医院感染率之间的关系。方法通过阶段性目标监测RICU医务人员洗手液和手消毒剂消耗用量、手卫生执行情况及医院感染发生率,研究医务人员手卫生依从性对医院感染发生率的影响。结果连续监测4个月,RICU医务人员洗手液和手消毒剂的每床日消耗用量由16.57ml提升至26.30ml;总体手卫生依从率有55.30%上升到91.04%,差异有统计学意义(P<0.01),其中使用洗手液和手消毒剂的手卫生依从率分别由21.18%、34.12%上升至38.81%、52.23%,差异均有统计学意义(P<0.01);RICU医院感染率由10.26%下降至2.63%(P<0.05);总体手卫生依从率和医院感染发生率呈负相关,有显著统计学意义(P<0.05)。结论持续性改进手卫生质量,可以提高RICU医务人员手卫生依从率,有效降低医院感染发生率,保证了医院感染管理质量。Objective To explore the relationship between hand hygiene quality improvement among medical staff and hospital infection rate in RICU. Methods The consumption amount of liquid soap and hand disinfectant,hand hygiene implementation and incidence of nosocomial infection in RICU were observed actively, then the effect of hand hygiene compliance rate on infection was evaluated. Results The consumption of liquid soap and hand disinfectant increased by 16.57 ml/bed · day to 26.30 ml/bed · day; the overall compliance rate of hand hygiene increased significantly, from 55.30% up to 91.04% after interventions （ P 〈0.01）,The hand hygiene compliance rates respectively increased from 21.18% and 34.12% to 38.81% and 52.23% by using liquid soap and hand disinfectant （ P 〈0.01） ;the hospital infection rate decreased from 10.26% to 2.63% （ P 〈0.05） ; there was a significantly negative correlation between hand hygiene compliance rate and incidence of hospital infection （ P 〈0.05）. Conclusion After the improvement of hand hygiene quality,the hand hygiene compliance rate increases,and the hospital infection reduces,so as to ensure the quality of hospital infection management.
