颅脑损伤患者急性期血清TNF-α和IL-1β水平的动态变化分析

摘　　要：目的旨在分析血清TNF-α(Tumor Necrosis Factor-α肿瘤坏死因子-α)和IL-1β(Interleukin-1β白介素-1β)在颅脑损伤急性期的变化及其临床意义。方法随机选取伤后24 h内入院的颅脑损伤患者30例,并选取17名健康人作为对照组,采用双抗体夹心ABC-ELISA法检测不同时间点(颅脑损伤后第1、3、7天)血清中TNF-α和IL-1β水平的变化。结果颅脑损伤后各时间点血清TNF-α和IL-1β的水平均高于正常对照组(p<0.05);伤后第1、3、7天的IL-1β、TNF-α水平的变化均无明显统计学差异。在各时间点上,重度组IL-1β、TNF-α水平均明显高于轻度组(p>0.05)。结论颅脑损伤患者急性期血清TNF-α、IL-1β水平明显升高,且血清TNF-α、IL-1β水平与损伤程度呈正相关,在临床上,血清TNF-α、IL-1β水平的监测对于颅脑损伤患者的病情程度能起到早期评估,并指导临床治疗的作用。Objective To explore the changes of serum TNF -α and IL - 1β and their clinical significance in pa- tients with traumatic brain injury acute phase. Methods The serum levels of TNF - α and IL - 1β were tested by ELISA method of double antibody with filling in 30 patients within 24h after traumatic brain injury randomly, and 17 cases of healthy people were used as controls. The concentration of serum TNF -α, IL - 1β were detected with ABC - ELISA method of doub- le antibody with fill after traumatic brain injury on 1,3, 7 d after injury. Results The serum TNF -α and IL - 1β levels of these patients were higher than normal control group at each time of detection, there was a significant difference （p 〈 0. 05 ）. But the serum TNF - αand IL - 1 β levels had not significant difference between each time on 1,3,7d after injury（p 〈 0. 05 ）. In the severe traumatic brain injury group, the levels of TNF - α, IL - 1β were significantly higher than those of gentle group （p 〈 0. 05 ）. Conclusion The serum TNF - α, IL - 1β levels in traumatic brain injury patients are increased significantly, and is in positive correlation with the levels of the injury. The serum TNF - α and IL - 1β also can reflect how severe the trau- matic brain injury is. Therefore, the dynamic observation of serum TNF -α and IL - 1β levels is of important value to judge the severity of patients with traumatic brain injury and guide the clinical treatment.
