SonoVue超声造影对不典型卵巢畸胎瘤的诊断价值

摘　　要：目的探讨SonoVue超声造影对不典型卵巢畸胎瘤的诊断价值。方法选择2009年11月至2013年7月于四川大学华西第二医院行常规超声检查发现卵巢囊性或囊实性病变而未明确诊断,而经术后病理学检查结果证实系成熟性卵巢畸胎瘤的12例患者的常规超声及超声造影检查资料为研究对象(本研究遵循的程序符合四川大学华西第二医院人体试验委员会所制定的伦理学标准,得到该委员会批准,并征得受试对象本人的知情同意,与之签署临床研究知情同意书)。结果常规超声结果示11例(99.2%)病灶内部呈囊实混合性回声,1例(0.8%)呈多房性囊性占位回声。SonoVue超声造影结果示10例(83.3%)患者病灶呈较均匀增强,2例(16.7%)为不均匀增强。SonoVue超声造影诊断符合率为83.3%(10/12),误诊率为16.7%(2/12)。结论对常规超声表现不典型的卵巢畸胎瘤,SonoVue超声造影仍有较特征性的肿瘤组织学和微循环特征,可为临床鉴别肿瘤来源与性质提供更为准确的诊断参考依据。Objective To explore the clinical values of contrast enhanced ultrasound on atypical ovarian teratoma by using SonoVue.Methods From November 2009 to July 2013,clinical dates of 12 women with atypical ovarian teratoma were observed.The study protocol was approved by the Ethical Review Board of Investigation in Human Being of West China Second Univercity Hospital.Informed consent was obtained from each participates.Results For the regular ultrasonic features,11 cases（99.2%）were mixed （cystic-solid）type with capsule formation,1 case （0.8%）were multilocular.The enhancement pattern of 10 cases （83.3%）were regular intensity,2 cases （16.7%）were irregular intensity.The diagnostic accordance rate of ultrasound contrast imaging Sonovue was 83.3%（10/12）,misdiagnosis rate was 16.7%（2/12）.Conclusions Because of ovarian teratoma′s characteristic pathologic components,even to those cases with atypical conventional ultrasound imaging,they still have characteristic contrast enhanced ultrasound features,such as histological pattern and microcirculation of the neoplasm.
