急性腰椎骨折并发椎间盘突出的最佳治疗方法分析

摘　　要：目的:总结分析急性腰椎骨折并发椎间盘突出的最佳治疗方法。方法:回顾分析本科住院患者99例急性腰椎骨折并发椎间盘突出患者,随机分为手术治疗组(A组)、微创手术治疗组(B组),保守治疗组(C组),每组33例。A组采用椎体间植骨融合术,B组采用微创手术治疗,C组采用电针治疗和牵引按摩进行保守治疗,对比分析3组治疗后的有效率、以及各项体征评分,术后1年定期随访,观察3组的生活质量评分。结果:B组的有效率、生活质量评分、以及各项体征评分与A、C组相比差异显著(P<0.05),具有统计学意义。B组的有效率、生活质量评分、以及各项体征评分均高于A、C组,A、C两组无显著性差异(P>0.05)。结论:微创手术与手术治疗和保守治疗方法相比,微创手术是治疗急性腰椎骨折并发椎间盘突出疾病的最佳方法。Objective：To determine on the optimal treatment for acute lumbar vertebral fracture complicated with protrusion of intervertebral disc through a retrospective analysis. Methods：99 patients with acute lumbar vertebral fracture complicated with protrusion of intervertebral disc admitted to Department of Surgery of our hospital were divided randomly into three groups ： open surgery group （ Group A, n = 33 ） , mini-invasive surgery group （Group B, n = 33） and conservative treatment group （ Group C, n = 33）. The patients in Group A underwent intervertebral bone grafting and fusion, the patients in Group B received a mini-invasive surgery and those in group C conservative treatment including electric acupuncture, traction and massage therapy. The effective rate, scores in signs and scores in quality of life at 1-year follow-up after treatment of the patients in three groups were compared. Results：The effective rate, scores in signs and scores in quality of life at 1-year follow-up after treatment of patients in Group B were significantly higher than those in Group A and Group C, reaching the level of statistical significance （ P 〈 0.05 ）. There was no significant difference in all these indexes between Groups A and C （ P 〉 0.05 ）. Conclusion： Compared with surgery and conservative treatment group, mini-invasive surgery is the optimal treatment for acute lumbar vertebral fracture complicated with protrusion of interv
