摘　　要：目的探讨应用腓浅动脉穿支皮瓣游离移植修复手部创面的临床效果。方法2008年3月至2012年3月，应用腓浅动脉穿支皮瓣游离移植修复手部创面共9例，其中手掌侧创面4例，虎口区创面1例，手指侧方创面1例，手背创面3例；缺损面积为3．0cm×4．5cm～5．0cm×10．0cm。其中4例与受区桡动脉头静脉吻合，4例与指总动脉浅静脉吻合，1例与尺动脉贵要静脉吻合。5例供区直接缝合，4例供区创面植皮修复。结果本组9例皮瓣全部存活，未出现血管危象。术后随访3—12个月，原缺损部位外形及功能恢复满意，供区外形功能无明显影响。结论应用腓浅动脉穿支皮瓣游离移植修复手部创面，临床效果满意，值得推广应用。Objective To investigate the clinical outcomes of repairing wound in the hand with free superficial peroneal artery perforator flap transplantation. Methods From March 2008 to March 2012, 9 cases of wound in the hand were treated with free superficial peroneal artery perforator flap transplantation. There were 4 cases of volar defects, 1 case of first web space wound, 1 case of lateral finger defect and 3 cases of dorsal defects. The size of the wounds ranged from 3.0 cmx 4.5 cm to 5.0 cm x10.0 cm. The perforator flap vascular pedicle was anastomosed to the radial artery and cephalic vein of the recipient area in 4 cases, to the proper digital artery and superficial vein in 4 cases, and to the ulnar artery and basilic vein in 1 case. The donor site was closed directly in 5 cases and covered with skin graft in 4 cases. Results All of the 9 /laps survived completely. No vascular crisis occurred. Postoperative follow-up ranged from 3 to 12 months. The appearance and function of the repaired sites were satisfactory. Conclusion Free superficial peroneal artery perforator flap transfer for coverage of wound in the hand can lead to satisfactory clinical results. It is an effective method that is worth wide clinical application.
